奈达铂对比顺铂联合同期放疗治疗中晚期宫颈癌的疗效和安全性的Meta分析

摘　　要：目的比较奈达铂对比顺铂联合同期放疗治疗中晚期宫颈癌的疗效与安全性。方法计算机检索PubMed、Cochrane Library、Embase、中国期刊网全文数据库(CNKI)、万方医学网等中英文数据库,检索时间为各数据库建库至2019年6月。收集奈达铂与顺铂联合同期放疗治疗中晚期宫颈癌的随机对照实验(RCTs),采用RevMan 5.3软件进行Meta分析。结果最终纳入6项研究共546例患者。奈达铂联合同期放疗与顺铂联合同期放疗的有效率和1年无瘤生存率比较,差异均无统计学意义(P>0.05);两组血液学毒性发生率比较,差异无统计学意义(P>0.05),但顺铂联合同期放疗组恶心呕吐、肾功能损害(Ⅰ、Ⅱ级)、急性放射性直肠炎(Ⅲ、Ⅳ级)的发生率均显著高于奈达铂联合同期放疗组(P<0.05)。结论奈达铂联合同期放疗与顺铂联合同期放疗治疗中晚期宫颈癌的疗效相当,而奈达铂联合同期放疗的不良反应发生率较低,但这一结论仍需多中心、高质量、大样本的长期研究进一步验证。Objective To evaluate the efficacy and safety of nedaplatin versus cisplatin combined with concurrent radiotherapy for advanced cervical cancer.Methods The randomized controlled trials about nedaplatin or cisplatin combined with concurrent radiotherapy for advanced cervical cancer published before June 2019 were collected from databases such as PubMed,Cochrane Library,Embase,CNKI,Wanfang data and so on.RevMan 5.3 was applied for meta-analysis.Results A total of 546 patients were involved in the selected six trials.There were no significant differences in the effective rate and the one year tumor-free survival rate between the two groups(P>0.05).There were yet no significant differences in hematological toxicity between the two groups(P>0.05).However,higher incidences of nausea and vomiting,renal damage(gradeⅠ-Ⅱ),and acute radiation proctitis(gradeⅢ-Ⅳ)were observed in cisplatin combined with concurrent radiotherapy group than in nedaplatin combined with concurrent radiotherapy group(P<0.05).Conclusion Nedaplatin combined with concurrent radiotherapy and cisplatin combined with concurrent radiotherapy were comparably effective in the treatment of advanced cervical cancer.But nedaplatin induced less adverse reactions than cisplatin.The above conclusions still require multi-center high-quality largescale long-term studies for further validation.
