脑出血患者血清8-OHDG、VILIP-1水平变化及临床意义

摘　　要：目的探讨血清8-羟基脱氧鸟苷(8-OHDG)、视椎蛋白样蛋白1(VILIP-1)水平与脑出血患者临床病理特征和预后的关系。方法选择118例脑出血患者(脑出血组)和100例健康志愿者(对照组)。检测两组血清8-OHDG、VILIP-1水平,分析血清8-OHDG、VILIP-1水平与脑出血患者年龄、性别、脑出血量、病情严重程度等临床病理特征以及预后的关系。结果脑出血组血清8-OHDG、VILIP-1水平高于对照组(P均<0.05)。大量出血、重型脑出血、预后不良患者血清8-OHDG、VILIP-1水平高于中量出血、中型脑出血、预后良好患者(P均<0.05),中量出血、中型脑出血患者血清8-OHDG、VILIP-1水平高于小量出血、轻型脑出血患者(P均<0.05)。脑出血患者血清8-OHDG水平与脑出血量、SSS评分呈正相关(r分别为0.462、0.421,P均<0.01),与GOS评分呈负相关(r=-0.519,P<0.01);血清VILIP-1水平与脑出血量、SSS评分呈正相关(r分别为0.436、0.426,P均<0.01),与GOS评分呈负相关(r=-0.504,P<0.01)。血清8-OHDG、VILIP-1、8-OHDG+VILIP-1预测脑出血患者预后不良的ROC曲线下面积分别为0.822、0.696、0.943,灵敏度分别为84.21%、71.93%、92.98%,特异度分别为81.97%、70.79%、95.08%。结论脑出血患者血清8-OHDG、VILIP-1水平升高,其水平与脑出血量、神经功能缺损程度和预后均有关,可作为脑出血患者预后评估的辅助指标。Objective To investigate the relationships of levels of serum 8-hydroxydeoxyguanosine(8-OHDG)and vertebral protein like protein 1(VILIP-1)with the pathological characteristics and prognosis of patients with cerebral hemorrhage.Methods Totally 118 patients with cerebral hemorrhage(cerebral hemorrhage group)and 100 healthy volunteers(control group)were selected.Serum levels of 8-OHDG and VILIP-1 were detected,and the relationships between the serum 8-OHDG and VILIP-1 levels and the pathological characteristics and prognosis of patients with cerebral hemorrhage were analyzed.Results The levels of serum 8-OHDG and VILIP-1 in the cerebral hemorrhage group were higher than those in the control group(P<0.05).The levels of serum 8-OHDG and VILIP-1 in patients with massive hemorrhage,severe cerebral hemorrhage and poor prognosis were higher than those in patients with moderate hemorrhage and moderate cerebral hemorrhage and good prognosis(both P<0.05).Serum levels of 8-OHDG and VILIP-1 in patients with moderate hemorrhage,moderate cerebral hemorrhage were higher than those in patients with minor hemorrhage and mild cerebral hemorrhage(both P<0.05).Pearson correlation analysis showed that serum 8-OHDG and VILIP-1 levels were positively correlated with cerebral hemorrhage and Scandinavian stroke scale(SSS)score(r=0.462,0.421,0.436,0.426,all P<0.05),and negatively correlated with Glasgow outcome(GOS)(r=-0.519,-0.504,all P<0.05).The area under the ROC curve(AUC)of serum 8-OHDG,VILIP-1,and 8-OHDG+VILIP-1 was 0.822,0.696,and 0.943,respectively,the sensitivities were 84.21%,71.93%,92.98%,and specificities were 81.97%,70.79%,and 95.08%,respectively.Conclusion Serum 8-OHDG and VILIP-1 levels increase significantly in patients with cerebral hemorrhage,and are related to the cerebral hemorrhage amount,the neurological impairment degree and the prognosis,which can be used as auxiliary indexes to evaluate the prognosis of patients with cerebral hemorrhage.
