摘 要:目的探讨中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、淋巴细胞与单核细胞比值(LMR)对乳腺癌骨转移患者预后的预测价值。方法回顾性分析2010年1月1日至2020年1月1日石河子大学医学院第一附属医院收治的100例初诊为乳腺癌骨转移患者的临床资料,采用受试者工作特征曲线确定NLR、PLR、LMR的最佳临界值,根据最佳临界值分组;比较不同NLR、PLR、LMR组患者年龄、肿瘤象限、左右侧、组织学分级、肿瘤T分期、肿瘤N分期、肿瘤直径、放化疗情况、生存率等,分析影响患者预后的因素。结果100例患者NLR、PLR、LMR最佳临界值分别为2.18、112.44、4.21。高NLR(NLR≥2.18)组、高PLR(PLR≥112.44)组患者3年生存率均明显低于低水平组,高LMR(LMR≥4.21)组患者3年生存率明显高于低水平组,差异均有统计学意义(P<0.05);LMR<4.21是乳腺癌骨转移患者预后不良的独立危险因素。结论低LMR是乳腺癌骨转移患者预后不良的独立危险因素。Objective To investigate the predictive value of the neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),lymphocyte-to-monocyte ratio(LMR)in the prognosis of the patients with bone metastasis of breast cancer.Methods The clinical data of 100 patients with bone metastasis of breast cancer in the First Affiliated Hospital of Medical College of Shihezi University from January 1,2010 to January 1,2020 were retrospectively analyzed.The receiver operating characteristic(ROC)curve was used to determine the optimal cut-off values of NLR,PLR and LMR.Then the age,tumor quadrant,left or right side,histological grade,tumor T stage,tumor N stage,tumor diameter,radiochemotherapy situation,survival rate,etc.were compared among different NLR,PLR and LMR groups.The factors affecting the prognosis were analyzed.Results Among 100 cases of breast cancer bone metastasis,the optimal cut-off values of NLR,PLR and PLR were 2.18,112.44 and 4.21 respectively.The 3-year survival rate in the high NLR(NLR≥2.18)group and high PLR(PLR≥112.44)group was significantly lower than that in the low NLR group and PLR group(P<0.05),and the 3-year survival rate in the high LMR(LMR≥4.21)group was significantly higher than that in the low LMR group,and the differences were statistically significant(P<0.05);LMR<4.21 was an independent risk factor for the poor prognosis of breast cancer bone metastasis.Conclusion Low LMR is an independent risk factor for the poor prognosis in the patients with breast cancer bone metastasis.
