肿瘤标志物联合炎症因子对乳腺癌患者的早期诊断价值

摘  要:目的探讨肿瘤标志物联合炎症因子对乳腺癌患者的早期诊断价值。方法随机选取2017年1月-2018年2月我院普外科收治的98例乳腺癌患者和100例乳腺癌良性患者,并筛选100例该时间段内于本院体检的健康女性。由病理检查结果将98例乳腺癌患者作为恶性组,100例乳腺癌良性患者作为良性组,100例体检健康女性为健康组。采取三组受试者静脉血,检测血清中血清糖类抗原125(CA125)、血清糖类抗原153(CA153)和癌胚抗原(CEA)肿瘤标志物及肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)等炎症因子水平。结果乳腺癌良性组和健康组肿瘤标志物水平比较无统计学差异(P>0.05)。乳腺癌恶性组血清中CA153、CA125、CEA水平均高于良性组和健康组(P<0.05)。乳腺癌良性组和健康组炎症因子水平比较无统计学差异(P>0.05)。乳腺癌恶性组血清中TNF-α、hs-CRP水平均高于良性组和健康组(P<0.05)。肿瘤标记物与炎症因子单独及联合检测对比,CA153+CA125+CEA+TNF-α+hs-CRP联合检测灵敏度及特异性最高,灵敏度为62.22%,特异性为92.00%。结论乳腺癌患者血清中的CA153、CA125、CEA等肿瘤标志物水平可能与乳腺癌转移及发展有关,TNF-α、hs-CRP等炎症因子水平可能与乳腺癌病理生理等功能有关。肿瘤标志物联合炎症因子检测对乳腺癌早期诊断和病情严重程度评估具有重要参考价值。Objective To investigate the early diagnostic value of tumor markers combined with inflammatory factor detection in patients with breast cancer.Methods Between January 2017 and February 2018,98 cases of breast cancer patients(malignant group),100 cases of benign breast cancer patients(benign group)and 100 cases of healthy women(healthy control group)in our hospital were randomly selected.Then the levels of serum carbohydrate antigen 125(CA125),carbohydrate antigen 153(CA153),carcinoembryonic antigen(CEA),tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP)and other inflammatory factors were measured in venous blood of three groups of subjects.Results The serum levels of CA153,CA125 and CEA in malignant group were higher than those in benign group and healthy control group(P<0.05),meanwhile the serum levels of TNF-α and hs-CRP in malignant group were higher than those in benign group and healthy control group(P<0.05).The levels of tumor markers and inflammatory factor showed no difference between benign group and healthy control group(P>0.05).Compared with the single detection,the sensitivity and specificity of combined detection of CA153+CA125+CEA+TNF-a+hs-CRP were the highest,62.22% and 92.00% respectively.Conclusion The levels of CA153,CA125,CEA and other tumor markers may be related to the metastasis and development of breast cancer,moreover,the levels of inflammatory factors such as TNF-α and hs-CRP may be related to the pathophysiology of breast cancer,so the combined detection of tumor markers and inflammatory factors has important reference value for early diagnosis and severity assessment of breast cancer.
