血清Hp抗体ELISA检测法诊断幽门螺杆菌感染的临床价值

摘　　要：目的探究血清幽门螺杆菌(Hp)抗体酶联免疫吸附测定法(ELISA)诊断Hp感染的临床价值。方法随机抽取我院200例疑似Hp感染者为研究对象,对其实施嗜银染色法、14C-尿素呼气试验法(14C-UBT)、血清Hp抗体ELISA检测法。以尿素酶试验和涂片染色联合检测为"金标准",比较上述3种检测方法的诊断结果。结果嗜银染色法诊断结果显示,170例被检测出Hp阳性,误诊、漏诊分别为2、4例,Kappa值为0.915;14C-UBT法诊断结果显示,167例被检测出Hp阳性,误诊、漏诊分别为7、12例,Kappa值为0.853;血清Hp抗体ELISA检测法诊断结果显示,169例被检测出Hp阳性,误诊、漏诊分别为6、9例,Kappa值为0.882。嗜银染色法的灵敏度高于14C-UBT法(P<0.05);嗜银染色法与血清Hp抗体ELISA检测法的灵敏度无显著差异(P>0.05);三种检测方法的特异度无显著差异(P>0.05);嗜银染色法的准确度高于14C-UBT法及血清Hp抗体ELISA检测法(P<0.05);血清Hp抗体ELISA检测法的灵敏度、特异度、准确度略高于14C-UBT法,但差异无统计学意义(P>0.05)。结论血清Hp抗体ELISA检测法诊断Hp感染具有较好的效果,且操作更简便。Objective To explore the clinical value of enzyme linked immunosorbent assay(ELISA)detection of serum Helicobacter pylori(Hp)antibody in the diagnosis of Hp infection.Methods A total of 200 cases suspected Hp infection in our hospital were randomly selected as the research objects.Silver staining,ELISA detection of serum Hp antibody and14 C urea breath test(14 C-UBT)were applied to the patients.Combined detection of urease test and smear staining were used as the"gold standard"to compare the diagnostic results of the above three detection methods.Results The results of silver staining showed that 170 cases were Hp positive,2 cases were misdiagnosed and 4 cases were missed diagnosis,Kappa value was 0.915.The results of14 C-UBT showed that 167 cases were Hp positive,7 cases were misdiagnosed and 12 cases were missed diagnosis,Kappa value was 0.853.The results of ELISA detection of serum Hp antibody showed that 169 cases were Hp positive,6 cases were misdiagnosed and 9 cases were missed diagnosis,Kappa value was 0.882.The sensitivity of silver staining was higher than that of14 C-UBT(P<0.05).There was no significant difference in sensitivity between silver staining and ELISA detection of serum Hp antibody(P>0.05).There were no significant differences in the specificity of the three methods(P>0.05).The accuracy of silver staining was higher than that of14 C-UBT and ELISA detection of serum Hp antibody(P<0.05).The sensitivity,specificity and accuracy of ELISA detection of serum Hp antibody were slightly higher than those of14 C-UBT,but the differences were not statistically significant(P>0.05).Conclusion ELISA detection of serum Hp antibody in the diagnosis of Hp infection has a better effect,and the operation is more simple.
