摘　　要：目的分析急性前循环大血管闭塞性脑卒中患者单核因子趋化蛋白(MCP)、钙黏蛋白(VEcadherin)及炎性因子的表达及临床意义。方法我院收治的153例急性前循环大血管闭塞性脑卒中患者(梗死组),根据患者临床症状分为进展性脑梗死61例和非进展性脑梗死92例,根据患者病灶可分为完全前循环梗死43例和不完全前循环梗死110例,根据神经功能损伤程度分为轻度61例、中度54例、重度38例。另选取同期健康体检者65例作为健康组。检测不同人群中MCP、VEcadherin及炎性因子的表达情况,以及不同情况下梗死组中MCP、VEcadherin及炎性因子的表达情况。结果MCP、VEcadherin、IL-6、TNF-ɑ、hs-CRP水平比较,梗死组高于健康组,进展性脑梗死组高于非进展性脑梗死组,完全前循环脑梗死组高于不完全前循环梗死组(P<0.05);不同程度神经损伤患者之间MCP、VEcadherin、IL-6、TNF-ɑ、hs-CRP水平有差异,重度神经损伤患者表达水平高于轻度和中度组(P<0.05)。结论急性前循环大血管闭塞性脑卒中患者MCP、VEcadherin及炎性因子的表达明显升高,其表达水平变化可为患者病情、临床治疗以及预后情况评估提供参考。Objective To analyze the expression and clinical significance of MCP,VE-cadherin and inflammatory factors in patients with acute occlusive stroke of great vessels of anterior circulation.Methods 153 patients with acute occlusive stroke of great vessels of anterior circulation admitted to our hospital were retrospectively analyzed(infarction group).According to the severity of the patients,they were divided into progressive cerebral infarction group(n=61),and non-progressive cerebral infarction(n=92).According to the patient's lesions,they were divided into complete infarction of anterior circulation group(n=43)and incomplete infarction of anterior circulation group(n=110).According to the degree of neurological impairment,they were divided into mild impairment group(n=61),moderate impairment group(n=54)and severe impairment group(n=38).Another 65 healthy people from our hospital in the same time were selected as health group.The expressions of MCP,VE-cadherin and inflammatory factors among different populations were compared,and the expressions of MCP,VE-cadherin and inflammatory factors in the infarction group under different conditions were compared.Results The expression levels of MCP,VE-cadherin,IL-6,TNF-ɑand hs-CRP in the infarction group were significantly higher than those in the healthy group(P<0.05).The expression levels of MCP,VE-cadherin,IL-6,TNF-ɑ,and hs-CRP of patients in the progressive cerebral infarction group were significantly higher than those in the non-progressive cerebral infarction group(P<0.05).The expression levels of MCP,VE-cadherin,IL-6,TNF-ɑ,and hs-CRP of patients in the complete infarction of anterior circulation group were significantly higher than those in the incomplete infarction of anterior circulation group(P<0.05).There were significant differences in the levels of MCP,VE-cadherin,IL-6,TNF-ɑ,and hs-CRP in patients with different degrees of nerve injury.The expression levels in patients with severe nerve injury were significantly higher than those with mild and moderate inju
