摘　　要：目的采用神经心理学量表评估无创性物理治疗加用多奈哌齐干预治疗Wernicke失语症恢复期的疗效。方法将60例Wernicke失语患者按随机数字表法分为三组各20例。前16周,联合治疗组与多奈哌齐组单用多奈哌齐进行干预,安慰剂组只用安慰剂治疗;第16~20周,联合治疗组和安慰剂组均使用低频rTMS进行治疗,多奈哌齐组继续单用药物干预。三组患者每一阶段治疗终点使用北京医院汉语失语症检查法评分。结果第16周,与安慰剂组相比,联合治疗组与多奈哌齐组失语总评分均增加(P<0.05)。第20周,与多奈哌齐组及安慰剂组比较,联合治疗组改善更明显(P<0.05)。结论单独使用多奈哌齐和低频rTMS均可改善脑卒中后Wernicke失语恢复期患者的言语功能,但多奈哌齐联合低频rTMS可达到最佳治疗效果。Objective To investigate the effect of donepezil combining with low frequency repetitive transcranial magnetic stimulation(rTMS)in the treatment of post-stroke Wernicke aphasia by using neuropsychological scale.Methods Sixty patients with Wernicke aphasia after stroke were randomly divided into 3 groups according to random number table method,20 in each group.In the first 16 weeks,the first group(combined treatment group)and the second group(donepezil group)was treated with donepezil,and the third group(placebo group)was treated with placebo.In the 16 th-20 th weeks,the first and third groups were treated with low-frequency rTMS while the second group used donepezil alone.The Chinese aphasia test was used at the end of each stage of treatment in the 3 groups.Results In the first 16 weeks,compared with the third group,the total aphasia scores of the first group and the second group were improved significantly(P<0.05).At the end of the 20 th week,compared with the second group and the third group,the combined treatment group was improved significantly(P<0.05).The 3 groups were significantly improved when compared with baseline(P<0.05).Conclusion s Both donepezil and low frequency rTMS can improve Wernicke aphasia severity,but the best outcomes were achieved when donepezil combining with low frequency rTMS.
