14C-尿素呼气试验与尿素酶试验在幽门螺杆菌感染诊断中的对比分析

摘　　要：目的探讨14C-尿素呼气试验(14C-UBT)和快速尿素酶试验(RUT)在幽门螺杆菌(Hp)感染诊断中的应用价值。方法2018年1月至2019年1月因上腹部不适在本院就诊的257例患者,均经14C-UBT和RUT检测。比较两种检测方法及检测准确性,不同消化道疾病Hp阳性检出率。结果257例患者中,14C-UBT和RUT检测均为阳性者108例,均为阴性119例,kappa值为0.766;14C-UBT HP阳性检出率为49.03%,RUT HP阳性检出率为46.69%,差异无统计学意义(P>0.05);14C-UBT检测约登指数为0.846,RUT检测约登指数为0.768,差异无统计学意义(P>0.05);14C-UBT和RUT在不同消化道疾病Hp阳性检出率比较,差异无统计学意义(P>0.05)。结论14C-UBT和RUT在Hp感染诊断方面均具有较高诊断价值,在不同消化道疾病Hp阳性检出率方面相似,临床上需根据14C-UBT和RUT不同特点选择合适的检测方法。Objective To compare the application value of carbon-14-urea breath test(14C-UBT)and rapid urease test(RUT)in the diagnosis of helicobacter pylori(Hp)infection.Methods Two hundred and fifty-seven patients who were admitted to the hospital for upper abdominal discomfort during the period from January 2018 to January 2019 were selected as subjects.All of them were given 14C-UBT and RUT.The two detection methods,detection accuracy and positive detection rate of Hp in different digestive tract diseases were compared.Results Of the 257 patients,108 were positive and 119 were negative for 14C-UBT and RUT,with a kappa value of 0.766.The positive detection rates of Hp by 14C-UBT and RUT were 49.03%and 46.69%,respectively(P>0.05).Youden indexes of 14C-UBT and RUT were about 0.846 and 0.768,respectively(P>0.05).There was no significant difference in the positive detection rate of Hp between 14C-UBT and RUT for different digestive tract diseases(P<0.05).Conclusion Both 14C-UBT and RUT are of great value in the diagnosis of Hp infection.They are similar in the positive detection rate of Hp in different digestive tract diseases.It is necessary to select appropriate detection methods according to the different characteristics of 14C-UBT and RUT.
