摘　　要：目的探讨经尿道膀胱肿瘤整块剜除术(ERBT)治疗非肌层浸润性膀胱癌(NMIBC)疗效及对血清金属蛋白酶-9(MMP-9)、Rho相关卷曲螺旋形成蛋白激酶2(Rock-2)、环氧合酶-2(Cox-2)水平的影响。方法我院诊治的150例NMIBC患者,按照随机数表法分为研究组与对照组各75例,分别给予ERBT和经尿道膀胱肿瘤电切术(TURBT)。比较两组围术期相关指标、手术前后血清MMP-9、Rock-2、Cox-2水平、术后并发症以及预后情况。结果研究组手术时间、膀胱冲洗时间、首次下地时间等围术期指标低于对照组(P<0.05);术前1 d及术后3 d两组间血清MMP-9、Rock-2、Cox-2水平比较,差异无统计学意义(P>0.05),术后3 d两组上述指标水平均降低(P<0.05);研究组术后并发症发生率及术后1年复发率均低于对照组(P<0.05)。结论ERBT可降低血清MMP-9、Rock-2、Cox-2水平,其治疗NMIBC的效果与TURBT相当,但该术式减少术后并发症,促进患者恢复,并降低术后复发风险,更具有优势。Objective To investigate the curative effect of enbloc resection of bladder tumor(ERBT)in the treatment of non-muscle invasive bladder cancer(NMIBC)and its influences on levels of serum matrix metalloproteinase-9(MMP-9),Rho associated coiled coil forming protein kinase 2(Rock-2)and cyclooxygenase-2(Cox-2).Methods A total of 150 NMIBC patients who were diagnosed and treated in our hospital were enrolled as research objects.They were divided into study or control group by random number table method,75 in each group.The two groups were treated with ERBT and transurethral resection of bladder tumor(TURBT),respectively.The perioperative related indexes,levels of serum MMP-9,Rock-2 and Cox-2 before and after surgery,postoperative complications and prognosis were compared between the two groups.Results The operation time,bladder irrigation time and the first leaving bed time in the study group were significantly shorter than those in the control group(P<0.05).Before 1 day of surgery and after 3 days of surgery,there were no significant differences in levels of serum MMP-9,Rock-2 and Cox-2 between the two groups(P>0.05).On the 3rd after surgery,levels of the above indexes in both groups were decreased(P<0.05).The incidence of postoperative complications and recurrence rate after 1 year of surgery in the study group were significantly lower than those in the control group(P<0.05).Conclusion ERBT can reduce the levels of serum MMP-9,Rock-2 and Cox-2,and its curative effect in the treatment of NMIBC is comparable to that of TURBT.However,the procedure is more advantageous in terms of reducing postoperative complications,promoting patients recovery and reducing postoperative recurrence risk.
