摘 要:目的了解四川省成都市0~6岁健康儿童血清25-羟维生素D25-hydroxy vitamin D,25-(OH)-D水平,为维生素D(vitamin D,Vit D)制剂补充提供依据。方法以2018年6月至2019年5月在成都市妇女儿童中心医院体检的9785名0~6岁健康儿童为研究对象,采用电化学发光法检测血清25-(OH)-D,比较不同性别、年龄、季节25-(OH)-D水平及不足与缺乏率有无差异。结果9785名儿童25-(OH)-D总体均值为(61.45±26.64)nmol/L,不足与缺乏率为36%;男女童均值无统计学差异(61.91±26.74)nmol/L,(60.85±26.49)nmol/L,不足与缺乏率女童略高(34.7%、37.7%);两者在不同年龄段均有统计学差异,其中3~6岁均值最低为(53.59±22.87)nmol/L;不足与缺乏率最高达45.9%,同年龄不同性别间无明显差异,而相同性别不同年龄段有统计学意义(P<0.05);夏季均值高于春,秋,冬三季,而不足与缺乏率降低。Vit D缺乏儿童临床主要表现为呼吸消化系统感染,肌肉骨骼疼痛及神经内分泌症状,占全部临床临床表现的61.1%。结论成都0~6岁儿童VitD缺乏较严重,女童,3~6岁儿童发生率更高,夏季以外其他季节都应注意补充。Objective To investigate the serum levels of 25-hydroxy vitamin D(25-(OH)-D in health children aged 0~6 years old in Chengdu city of Sichuan province in order to provide evidence for vitamin D(Vit D)supplement.Methods A total of 9785 children were enrolled from June 2018 to May 2019.Serum 25-(OH)-D were measured by electrochemilumininescence.Their mean values and deficiency in between genders,ages and seasons were compared.Results The mean value of 25-(OH)-D in all 9785 samples was(61.45±26.64)nmol/L and the prevalence of Vit D deficiency was 36%.There was no significant difference in the mean value of Vit D between boys and girls[(61.91±26.74)nmol/L vs.(60.85±26.49)nmol/L].However,insufficient proportion in the girls was higher than that in the boys(34.7%vs.37.7%).The mean values and deficiency were significantly different among different ages in which children aged 3~6 years old had the lowest mean value of Vit D[(53.59±22.87)nmol/L]and the highest deficiency(45.9%).The mean value and insufficient proportion were not significantly different between boys and girls at the same age,but there were different among different ages at the same genders(P<0.05).The mean value in summer was higher than that in spring,autumn and winter.In contract,the Vit D deficiency was decreased in summer when compared to other seasons.The main clinical manifestations of Vit D deficiency were respiratory infection and digestive infection,musculoskeletal diseases and neuroendocrine diseases in almost 61.1%of children with Vit D deficiency.Conclusion s Vit D nutrition status of infants at 0~6 years old in Chengdu is not very well.Girls and children aged 3~6 years have more deficiency rates.More attention should be paid to Vit D supplement in spring,autumn and winter.
