Trop2在胃癌组织中的表达及其临床意义的Meta分析

摘　　要：目的:系统评价肿瘤相关钙信号转导因子2(Trop2)在胃癌组织中的表达水平及其与临床病理特征之间的关系。方法:检索PubMed、Cochrane Library、Web of Science、万方数据库(WanFang)、中国知网数据库(CINK)、维普数据库(VIP),查找建库至2019年3月31日公开发表的关于Trop2与胃癌及其临床病理特征关系的相关研究,按纳入和排除标准筛选文献,用RevMan5.3软件进行统计学分析。结果:最终纳入6篇文献,合计胃癌组865例,对照组194例。Meta分析结果显示:Trop2在胃癌组和对照组[OR=9.18,95%CI(2.97,28.37),P=0.0001]、低分化组和高中分化组[OR=1.73,95%CI(1.28,2.35),P=0.0004]、临床分期Ⅰ~Ⅱ期组与Ⅲ~Ⅳ期组[OR=0.43,95%CI(0.31,0.57),P<0.00001]、淋巴结转移组和无淋巴结转移组[OR=2.00,95%CI(1.52,2.62),P<0.00001]、未达浆膜组和侵达浆膜组[OR=0.35,95%CI(0.26,0.47),P<0.00001]、有远处转移组和无远处转移组[OR=3.62,95%CI(1.58,8.31),P=0.002]表达差异均有统计学意义。但在肠型胃癌组和弥漫型胃癌组[OR=1.74,95%CI(1.00,3.03),P=0.05]、男性组和女性组[OR=1.00,95%CI(0.59,1.70),P=1.00]、<50岁组和≥50岁组[OR=1.24,95%CI(0.67,2.30),P=0.48]、幽门螺杆菌阳性组和阴性组[OR=3.35,95%CI(0.87,12.98),P=0.08]表达差异均无统计学意义。结论:胃癌组织中Trop2呈高表达,高表达的Trop2在胃癌的发生及细胞分化过程中发挥重要作用,并可促进癌细胞发生淋巴结转移及远处转移。Objective:To investigate the expression and clinicopathologic features of Trop2 in gastric cancer with a systematic review.Methods:Literatures were searched in PubMed,Cochrane Library,Web of Science,CNKI,VIP and WanFang databases to retrieve studies published on the relationship between Trop2 and gastric cancer and its clinicalpathologic characteristics.Screened literatures according to inclusion and exclusion criteria,and statistical analysis was conducted using RevMan5.3 software.Results:A total of 6 articles including 865 cases in gastric cancer group and 194 cases in the control group.The results of meta-analysis indicated that there were significant differences in positive rate of Trop2 expression between gastric cancer tissues and normal gastric tissues[OR=9.18,95%CI(2.97,28.37),P=0.0001],Low differentiation group and high differentiation group[OR=1.73,95%CI(1.28,2.35),P=0.0004],Clinical stageⅠ~Ⅱgroup andⅢ~Ⅳgroup[OR=0.43,95%CI(0.31,0.57),P<0.00001],Lymph node metastasis and non-lymph node metastasis[OR=2.00,95%CI(1.52,2.62),P<0.00001],unreaching serosa group and invading serosa group[OR=0.35,95%CI(0.26,0.47),P<0.00001],distant metastases and no distant metastases[OR=3.62,95%CI(1.58,8.31),P=0.002].No significant difference was found between intestinal-type gastric cancer and diffuse-type gastric cancer[OR=1.74,95%CI(1.00,3.03),P=0.05],Male and female[OR=1.00,95%CI(0.59,1.70),P=1.00],<50 years old and≥50 years old[OR=1.24,95%CI(0.67,2.30),P=0.48],Helicobacter pylori positive and negative group[OR=3.35,95%CI(0.87,12.98),P=0.08].Conclusion:Trop2 is highly expressed in gastric cancer.High expression of Trop2 plays an important role in the carcinogenesis and differentiation of gastric cancer,and can promote lymph node metastasis and distant metastasis of gastric cancer cells.
