美学区单颗牙即刻种植即刻修复与延期修复的临床效果比较

摘　　要：目的比较美学区单颗牙即刻种植后即刻修复与延期修复的临床效果。方法选取2016年6月至2018年7月在郑州大学第一附属医院口腔种植科接受美学区单颗牙即刻种植的44例患者,按照随机数表法分为A组和B组,各22例。A组即刻种植后当天完成种植体支持式临时冠修复,3个月后永久修复。B组接受即刻种植后3个月行种植体支持式临时冠修复,临时修复后3个月永久修复。于永久修复后1 a比较两组种植体存留率、种植体边缘骨吸收(MBL)、美学效果[红色美学评分(PES)]及患者满意度[视觉模拟评分量表(VAS)评分]。结果两组种植体存留率均为100%。在永久修复后1 a,两组MBL和PES评分比较,差异无统计学意义(均P>0.05)。A组VAS评分高于B组(P<0.05)。结论美学区单颗牙即刻种植后即刻修复与延期修复的种植体存留率和对软硬组织的影响均无显著差异,即刻修复患者满意度更高。Objective To compare the clinical effects of immediate and delayed implant-supported restoration following immediate implant placement of single tooth in the aesthetic zone.Methods A total of 44 patients with single tooth immediate implant in the aesthetic zone in the Department of Oral Implantology of the First Affiliated Hospital of Zhengzhou University from June 2016 to July 2018 were selected.They were divided into group A and group B according to the method of random number table,and each group had 22 cases.In group A,immediate implant-supported temporary crown was completed immediately after implantation,and the definitive restoration was completed 3 months later.Group B received implant-supported temporary crown at 3 months after immediate implant placement,and the definite restoration was completed at 3 months after provisional restoration.The implant survival rate,marginal bone loss(MBL),aesthetic effect[pink esthetic score(PES)]and patients’satisfaction[visual analogue scale(VAS)]were compared between the two groups at 1 year after definite restoration.Results The implant survival rates of the two groups were both 100%.At 1 year after permanent restoration,there were no statistical differences on MBL score and PES score between the two groups(both P>0.05).The VAS score of group A was higher than that of group B(P<0.05).Conclusion There are no statistical differences on the implant survival rate and effect on soft and hard tissue between immediate implant-supported restoration and delayed implant-supported restoration following immediate implant placement of single tooth in the aesthetic zone,and the patients’satisfaction of immediate irestoration is higher than that of delayed restoration.
