细胞角蛋白片段19的表达在尿路上皮癌诊断中的价值

摘　　要：目的探讨细胞角蛋白片段19(Cytokeratin 19 fragment,CYFRA211)的表达在尿路上皮癌诊断中的价值。方法回顾性分析2015年1月至2019年9月我院收治的76例尿路上皮癌患者的血清CYFRA211水平及临床病理资料,另选取同期收治的泌尿系良性疾病者81例为对照组,比较两组血清CYFRA211阳性率。结果对照组CYFRA211阳性率为9.88%(8/81);观察组CYFRA211阳性率为39.47%(30/76),明显高于对照组(χ^2=18.72,P<0.01)。上尿路尿路上皮癌中血清CYFRA211阳性率(13/17)高于膀胱尿路上皮癌(17/59),差异有统计学意义(χ^2=12.55,P<0.01);膀胱癌中高级别尿路上皮癌患者血清CYFRA211阳性率(14/31)明显高于低级别尿路上皮癌(2/27)(χ^2=10.30,P<0.01);膀胱癌中低级别尿路上皮癌患者血清中CYFRA211阳性率与对照组无明显统计学差异(P>0.05)。结论血清CYFRA211的表达与尿路上皮癌的发生有关,可作为诊断尿路上皮癌的生物学指标。Objective To study the value of detection of serum cytokeratin 19 fragment(CYFRA211)in the diagno⁃sis of urothelial carcinoma(UC).Methods The retrospective analysis was performed on 76 UC patients and 81 cases with benign diseases of the urinary system in our hospital from January 2015 to September 2019.Serum CYFRA211 levels and histopathological data were analyzed.Results Compared with control group(8/81),serum CYFRA211 positive rate in UC patients(30/76)was significantly higher(χ^2=18.72,P<0.01).The positive rate in upper urinary tract UC(13/17)was also higher than that in bladder UC patients(17/59)(χ^2=12.55,P<0.01);In urothelial bladder carcinoma patients,the serum CYFRA211 positive rate in high-grade urothelial bladder carcino⁃ma(14/31)was higher than that in low-grade urothelial bladder carcinoma(2/27)(χ^2=10.30,P<0.01).Howev⁃er,no significant difference of serum CYFRA211 positive rate was observed between low-grade urothelial bladder carcinoma and control group(P>0.05).Conclusion Serum CYFRA211 level is associated with the occurrence of UC,which may be a potential marker of UC.
