鼻内窥镜下鼻腔泪囊吻合术与外路鼻腔泪囊吻合术治疗急性泪囊炎的临床疗效对比

摘　　要：目的比较鼻内窥镜下鼻腔泪囊吻合术及外路鼻腔泪囊吻合术治疗急性泪囊炎的临床疗效。方法回顾性分析2012年2月至2014年10月在济南市明水眼科医院手术的67例67眼急性泪囊炎的患者,分2种方法进行治疗,鼻内窥镜下鼻腔泪囊吻合术组(endoDCR组)在全身应用抗生素的同时,急性期行鼻内窥镜下鼻腔泪囊吻合术,外路鼻腔泪囊吻合术组(extDCR组)首先全身应用抗生素,炎症控制2周后行外路鼻腔泪囊吻合术,两组患者术后定期泪道冲洗,随访12个月,比较两组患者的疗效。结果endoDCR组:治愈35例35眼,好转2例2眼,无效0例0眼,总有效率为100%。extDCR组:治愈28例28眼,好转1例1眼,无效1例1眼,总有效率为96.66%。两组患者术后有效率比较,差异无统计学意义(P=0.448>0.05)。结论在急性泪囊炎急性期即可行鼻内窥镜下鼻腔泪囊吻合术且疗效肯定,与炎症控制后再行外路鼻腔泪囊吻合术疗效相当。Objective To compare the clinical curative effect of nasal endoscopic dacryocystorhinostomy and external dacryocystorhinostomy on acute dacryosystitis.Methods We retrospectively analyzed a series of 67 cases(67 eyes)with acute dacryosystitis who treated in Jinan Mingshui Eye hostipal from February 2012 to October 2014.All patients were divided into two groups.Nasal endoscopic dacryocystorhinostomy group(endoDCR group)received systemic antibiotic therapy and nasal endoscopic dacryocystorhinostomy in acute stage.External dacryocystorhinostomy group(extDCR group)underwent external dacryocystorhinostomy after two weeks of anti-infection therapy.All patients received postoperative irrigation periodically.Therapeutic effects of two methods were compared after 12 months of follow-up.Results In endoDCR group,37 cases(37 eyes)were cured,2 cases(2 eyes)was improved,and the effective rate was 100%.In extDCR group,28 cases(28 eyes)were cured,1 case(1 eye)was improved and 1 case(1 eye)was invalid,and the effective rate was 96.66%.The postoperative effective rate of the two groups was not statistically significant(P=0.448>0.05).Conclusion Nasal endoscopic dacryocystorhinostomy could be performed in the acute phase of acute dacryosystitis.That method has the similar curative effect with external dacryocystorhinostomy followed by infection control.
