摘 要:目的探讨常规超声检查、超声弹性成像检查及钼靶X线检查联合应用对三阴性乳腺癌(TNBC)的诊断价值。方法选取2014年7月至2018年7月磐安县人民医院收治的经术后病理检查证实为TNBC的患者85例(TNBC组),另选取同期良性乳腺疾病患者85例作为对照组,通过多模态影像技术对TNBC患者进行检查,并与良性乳腺疾病进行比较,总结TNBC的影像学表现,同时分析多模态影像技术对TNBC的诊断准确率。结果85例TNBC患者中,出现腋窝淋巴结转移56例(65.88%);乳腺浸润性导管癌64例(75.29%);导管内原位癌合并浸润性导管癌12例(14.12%)。钼靶X线检查与常规超声检查在TNBC病灶形态、边界清晰度方面比较差异均无统计学意义(均P>0.05);钼靶X线检查肿块中有微钙化者明显多于常规超声检查,差异有统计学意义(P<0.05)。多模态影像技术对TNBCⅣ~Ⅵ类病灶的诊断准确率均高于单独应用钼靶X线检查和常规超声检查联合超声弹性成像检查,差异均有统计学意义(均P<0.05)。结论多模态影像技术对TNBC的诊断率最高,临床应用能够有效降低TNBC的漏诊率。Objective To explore the clinical value of ultrasonography,elastography and molybdenum target X-ray examinations in the diagnosis of triple negative breast cancer(TNBC).Methods A total of 85 patients with TNBC confirmed by postoperative pathology admitted in Pan'an County People's Hospital from July 2014 to July 2018 were enrolled(study group),and another 85 patients with benign breast diseases admitted in the same period served as control group.Two groups underwent ultrasonography,elastography and molybdenum target X-ray examinations and the diagnostic accuracy of multimodal imaging techniques was evaluated.Results Among 85 TNBC patients,there were 56 cases with axillary lymph node metastasis(65.88%),64 cases of invasive ductal carcinoma(75.29%),and 12 cases of in situ cancer combined with invasive ductal carcinoma(14.12%).There was no significant difference in the proportion of regular shape and clear boundary between molybdenum target X-ray and ultrasound examination(both P>0.05).Mmolybdenum target X-ray detected more patients with calcification of the lumps than ultrasound examination(P<0.05).The diagnosis accuracy of multimodal imaging techniques for TNBC was significantly higher than that of molybdenum target X-ray examination or ultrasound examination combined with elastography(both P<0.05).Conclusion Multimodal imaging technique has higher diagnostic accuracy for TNBC,which can effectively reduce the missed clinical diagnosis rate of TNBC.
