不同提上睑肌手术方法矫正先天性上睑下垂的临床研究

摘　　要：目的探讨应用不同提上睑肌手术方法治疗先天性上睑下垂。方法自2015年1月至2017年1月我院收治的上睑下垂患者103例共183只眼,根据提上睑肌肌力大小分别采用提上睑肌折叠后垂直缝合睑板、提上睑肌折叠后水平缝合睑板、提上睑肌缩短术或提上睑肌缩短+联合筋膜鞘悬吊术(conjoint fascial sheath CFS)四种手术方式。结果术后回访6个月至2年,正矫共166只眼,欠矫12只眼,过矫5只眼,治愈率为90.71%。结论根据提上睑肌肌力大小可分为:肌力≤4mm,可选用提上睑肌缩短或提上睑肌缩短联合CFS悬吊术;肌力5-7mm,可选用单纯提上睑肌缩短术;肌力8-10mm,可选用提上睑肌缩短折叠后水平固定睑板;肌力≥10mm可选用提上睑肌折叠后垂直固定睑板,针对患者肌力选择不同提上睑肌手术矫正上睑下垂,手术设计个性化,科学化,术后效果满意。Objective To explore the treatment of congenital blepharoptosis with different levator muscle surgery methods.Methods From January 2015 to January 2017,a total of 183 eyes of 103 patients with drooping blepharoplasty were treated in our hospital.According to the strength of the levator muscle,the upper eyelid muscle was folded and the tarsal plate was sutured vertically.After the eyelid muscles are folded,the eyelids are sutured horizontally,the upper eyelid is shortened or the upper eyelid is shortened+conjoint fascial sheath CFS.Results After 6 months to 2 years of postoperative follow-up,95 cases were corrected 166 eyes in total,3 cases were undercorrected in 12 eyes,3 cases were overcorrected in 5 eyes,2 cases were uncorrected in 2 eyes,and the cure rate was Is 90.71%.Conclusion According to the strength of upper levator muscle strength,it can be divided into:muscle strength≤4mm,upper levator muscle shortening or upper levator muscle shortening combined with CFS suspension can be selected;muscle strength 5~7mm,optional upper eyelid lifting Muscle shortening;muscle strength 8-10mm,optional upper eyelid muscle shortening can be used to fix the eyelid horizontally after folding;muscle strength≥10mm can be used to vertically fix the eyelid after folding the upper eyelid muscle,choose different upper eyelids according to the muscle strength Muscle surgery to correct ptosis,the surgical design is personalized and scientific,and the postoperative results are satisfactory.
