全瓷基台与纯钛基台在前牙缺失全瓷冠修复中的比较

摘　　要：目的比较全瓷基台与纯钛基台在前牙缺失全瓷冠修复中的应用效果。方法选取我院前牙缺失患者88例(2018年9月~2019年9月),共95颗前牙缺失,依据修复基台不同分为纯钛组(n=44,46枚种植体)、全瓷组(n=44,49枚种植体)。两组均行全瓷冠修复,纯钛组采用纯钛基台,全瓷组采用全瓷基台。比较两组修复后1个月、3个月、6个月牙龈指数(GI)、菌斑指数(PLI)、修复满意度、修复前、修复后6个月口腔健康影响程度量表(OHIP-14)评分。结果修复后1个月,两组GI、PLI、修复满意度比较无明显差异(P>0.05),修复后3个月、6个月,全瓷组GI、PLI低于纯钛组,修复满意度高于纯钛组(P<0.05);修复后6个月,全瓷组OHIP-14评分低于纯钛组(P<0.05)。结论相较于纯钛基台,全瓷基台应用在前牙缺失全瓷冠修复中,可降低对口腔健康的影响,提高修复满意度与生活质量。Objective To compare the effect of all-ceramic abutment and pure titanium abutment in the restoration of an allceramic crown with missing anterior teeth.Methods 88 patients with missing anterior teeth(September 2018 to September 2019)were selected in our hospital,a total of 95 anterior teeth were missing.According to different restoration abutments,they were divided into pure titanium group(n=44,46 implants)and all-ceramic group(n=44,49 implants).Both groups were repaired with all-ceramic crowns.The pure titanium group used pure titanium abutments and the all-ceramic group used all-ceramic abutments.Comparing the gingival index(GI),plaque index(PLI)at 1 month,3 months and 6 months after repair,repair satisfaction and the scores of the oral health impact scale(OHIP-14)before and 6 months after repair of the two groups.Results One month after repair,there was no significant difference in GI,PLI and repair satisfaction between the two groups(P>0.05),3 months and 6 months after repair,the GI and PLI of the all-ceramic group were lower than the pure titanium group,and the repair satisfaction was higher than that of the pure titanium group(P<0.05);six months after repair,the OHIP-14 score of the all-ceramic group was lower than that of the pure titanium group(P<0.05).Conclusion Compared with pure titanium abutments,the application of all-ceramic abutments in the restoration of anterior teeth with all-ceramic crowns can reduce the impact on oral health and improve the restoration satisfaction and quality of life.
