血滞通胶囊联合阿托伐他汀对老年非酒精性脂肪肝病人肝纤维化的影响以及疗效分析

摘　　要：目的观察血滞通胶囊联合阿托伐他汀治疗老年非酒精性脂肪肝(NAFLD)的临床疗效,以及对肝纤维化的影响。方法选取2017年2月至2019年1月我院诊疗的老年NAFLD病人116例,按随机数表法分为对照组和观察组,每组58例。对照组给予阿托伐他汀,10 mg/次,1次/d,观察组在对照组基础上加用血滞通胶囊,2粒/次,3次/d,4周/疗程,连续3个疗程。比较2组血脂、肝功能、肝纤维化指标、氧化应激指标和炎症因子水平,以及临床疗效和不良反应。结果疗程结束后,观察组TC、TG、LDL-C、AST、ALT、透明质酸酶(HA)、Ⅲ型前胶原(PCⅢ)、Ⅳ型胶原(Ⅳ-C)、层黏连蛋白(LN)、丙二醛(MDA)、IL-6和TNF-α显著低于对照组(P<0.05),而HDL-C和超氧化物歧化酶(SOD)显著高于对照组(P<0.05)。观察组临床治疗有效率为91.4%(53/58),显著低于对照组的72.4%(42/58)(P<0.05)。观察组和对照组的不良反应发生率分别为10.3%(6/58)和8.6%(5/58),2组间差异无统计学意义(P>0.05)。结论血滞通胶囊联合阿托伐他汀治疗老年NAFLD,可以抑制氧化应激炎症反应,发挥抗肝纤维化作用,临床疗效显著,使用安全。Objective To explore the effects of Xuezhitong capsule combined with atorvastatin on liver fibrosis in the elderly patients with non-alcoholic fatty liver disease(NAFLD).Methods A total of 116 elderly patients with NAFLD were enrolled in this study,and they were divided into control group(n=58)and observation group(n=58).The control group received atorvastatin(10 mg per time,1 time per day),and the observation group received atorvastatin(10 mg per time,1 time per day)and Xuezhitong capsule(0.9 g per time,3 times per day)for 12 weeks.The levels of blood lipid,liver function,liver fibrosis,oxidative stress,inflammation factors,clinical efficacy and adverse reactions rate were compared between the two groups.Results After treatment,the levels of total cholesterol,triglyceride,low density lipoprotein-cholesterol,aspartate transaminase,alanine aminotransferase,hyaluronic acid(HA),procollagenⅢ(PCⅢ),typeⅣcollagen(Ⅳ-C),laminin(LN),malondialdehyde(MDA),interleukin-6,and tumor necrosis factor-αin observation group were significantly lower than those in control group(P<0.05),and the levels of high density lipoprotein-cholesterol and superoxide dismutase(SOD)were significantly higher than those in control group(P<0.05).The clinical effective rate in observation group was 91.4%(53/58),compared with 72.4%(42/58)in control group(P<0.05).The adverse reactions rate in observation group and control group was 10.3%(6/58)and 8.6(5/58),with no significant difference(P>0.05).Conclusions Xuezhitong capsule combined atorvastatin can inhibit oxidative stress and inflammatory reaction,as well as resist fibrosis in the elderly patients with NAFLD,with significant clinical efficacy and safety.
