单尖法填充对离体下颌磨牙的封闭效果研究

摘　　要：目的研究单尖法在牙齿根管填充治疗中的封闭效果。方法于2017年6月—2019年6月间,收集重度牙周炎患者下颌离体磨牙100例作研究样本。取其中20例作为对照组并再次均分两组,其中一组为染色法阳性对照组,不予指甲油涂布,另一组为阴性对照组,整体连根尖孔区域涂布指甲油两遍。剩余80例患牙作实验组,亦予均分两组,一组行单尖法充填,40例作单尖法组;另一组行热垂直加压法充填,40例作热加压组。予以相应根管充填治疗后,以微渗漏实验比较各组填充的封闭效果。结果90 d观察期内,单尖法组细菌根尖微渗漏发生率达45.00%,热加压组微渗漏发生率达50.00%,两组对比差异无统计学意义(P>0.05);单尖法组平均微渗漏时间为(65.73±17.48)d,热加压组平均微渗漏时间为(60.22±16.83)d,两组对比差异无统计学意义(P>0.05)。单尖法组根管填充时间显著低于热加压组,差异有统计学意义(P<0.05)。结论以iRoot SP根管封闭剂为填充基础,本次研究单尖法与热垂直加压法治疗相近,在微渗漏发生率及平均微渗漏时间上二组差异不显著,牙根封闭效果相近。但单尖法临床实施所需耗时更短,为治疗节约大量时间,提升了治疗效率,缩短患者就诊及张口时间。推荐在根管充填治疗中采用单尖法。Objective To study the sealing effect of single point method in root canal filling.Methods From June 2017 to June 2019,100 molars from patients with severe periodontitis were collected as study samples.Take 20 of them as control group and divided them into two groups again.Among them,one group was positive staining control group,no nail polish coating was applied,and the other was negative control group,and nail polish was coated with nail polish for two times.The remaining 80 teeth were divided into two groups:one group was filled with single point method,40 with single point method,and the other group was filled with vertical hot pressing method,and 40 with hot pressing method.After the root canal filling treatment,the sealing effect of each group was compared by microleakage experiment.Results During the 90 days observation period,45.00%of the bacteria in the single tip method group and 50.00%in the hot compression group had microleakage,and there was no significant difference between the two groups(P>0.05).The mean time of microleakage was(65.73±17.48)d in the single point method group and(60.22±16.83)d in the hot compression group,and there was no significant difference between the two groups(P>0.05).The filling time of single point method group was significantly lower than that of hot pressing group(P<0.05).Conclusion Based on root iRoot SP root canal sealer,the single point method is similar to the hot vertical compression method in this study.There is no significant difference in the incidence of microleakage and the average microleakage time between the two groups,and the root sealing effect is similar.However,the single point method needs less time for clinical implementation,which saves a lot of time for treatment,improves the treatment efficiency,and shortens the time for patients to see a doctor and open their mouth.Single point method is recommended in root canal filling.
