摘 要:目的通过观察叉头框蛋白O亚型3a(Foxo3a)和p27激酶蛋白抑制剂1(p27 kip1)在电针治疗面神经压榨性损伤过程中的表达变化,探讨电针促进面神经再生可能的作用机制。方法将42只雄性SD大鼠按照随机数字表法分为3组。正常组6只,不做任何处理;模型组18只,建立右侧面神经上颊支压榨损伤模型;电针组18只,建立右侧面神经上颊支压榨损伤模型后予穴位电针治疗。模型组、电针组均于术后4、14、28 d分别随机抽取6只大鼠取材。采用苏木素-伊红染色观察面神经元的形态变化,蛋白免疫印迹法及免疫组化法检测面神经元中Foxo3a及p27 kip1的表达,进行统计分析。结果与模型组比较,电针组面神经元在术后各时间点细胞团更聚集,边界更清晰,细胞数目、神经元突起增多。术后模型组及电针组Foxo3a、p27 kip1蛋白表达量及免疫组化阳性细胞数均呈现出先下降后上升的趋势。术后4、14 d与正常组比较,模型组和电针组Foxo3a、p27 kip1蛋白表达量及免疫组化阳性细胞数均低于正常组(P<0.05),且电针组低于模型组同期(P<0.05)。术后第28 d,模型组、电针组Foxo3a蛋白表达量及免疫组化阳性细胞数与正常组比较差异均无统计学意义(P>0.05);术后28 d,电针组p27 kip1蛋白表达低于正常组(P<0.05),p27 kip1免疫组化阳性细胞数低于模型组同期(P<0.05)。结论电针能加快面神经压榨性损伤的修复,其机制可能与促进面神经核团中Foxo3a、p27 kip1的表达量下降、缩短细胞周期有关。Objective To explore the possible mechanism of electroacupuncture promoting peripheral facial nerve regeneration by observing the change in the expression of forkhead box protein o subtype 3a(Foxo3a)and p27 kinase protein inhibitor 1(p27 kip1)in the process of peripheral facial nerve crush injury treated with electroacupuncture.Methods Forty two male Sprague-Dawley rats were divided into three groups according to the random number table method.The 6 rats in the normal group didn't receive any treatment,facial nerve crush injury model for right superior buccal branch was set up for 18 rats in the model group,and 18 rats in the electroacupuncture group was treated with acupoint electroacupuncture after facial nerve crush injury model for right superior buccal branch was established.Six rats were randomly selected from the model group and the electroacupuncture group respectively for sampling on the 4th,14th and 28th days after operations.The morphological changes in facial neurons were observed by hematoxylin eosin staining,the expression of Foxo3a and p27 kip1 in facial neurons was detected by Western blotting and immunohistochemistry,and the results were statistically analyzed.Results Compared with the model group,the number of cells and the number of neurites increased in EA group at each time point after operation.The number of neurons was more clear and the boundary was more clear.The expression of FOXO3a,p27 kip1 protein and the number of immunohistochemical positive cells in the model group and electroacupuncture group decreased at first and then increased.Compared with the normal group,the expression of FOXO3a,p27 kip1 protein and the number of immunohistochemical positive cells in the model group and electroacupuncture group were lower than those in the normal group(P<0.05),and the electroacupuncture group were lower than the model group at the same period(P<0.05).On the 28th day after operation,there was no significant difference in FOXO3a protein and immunohistochemical positive cells between the model g
