摘 要:目的研究高血压脑出血的发病是否与同型半胱氨酸、尿酸的变化有关。方法连续收集2018年1月~2019年10月就诊于天水市第一人民医院神经内一科的高血压脑出血患者115例和高血压患者120例;高血压脑出血患者根据出血量多少分成三组,小量出血组(<10 mL)60例,中量出血组(10~30 mL)35例,大量出血组(>30 mL且<60 mL)20例。全部入组患者就诊24 h内行同型半胱氨酸、尿酸测定。比较高血压脑出血患者和高血压患者同型半胱氨酸、尿酸的变化,评估其对高血压脑出血发病的影响。结果高血压脑出血患者和高血压患者同型半胱氨酸、尿酸的变化,差异均有统计学意义(P<0.05);小量出血组、中量出血组同型半胱氨酸、尿酸的增高居多,大量出血组偏少,表明同型半胱氨酸、尿酸虽敏感性较高,但其数据变化与脑出血量的多少关系不大。结论同型半胱氨酸、尿酸均是高血压脑出血的高危因素,对高血压患者定期监测同型半胱氨酸、尿酸,给予适当干预,可降低其发生脑出血的风险。Objective To study whether the onset of hypertensive cerebral hemorrhage is related to the changes of homocysteine and uric acid.Methods 115 patients with hypertensive cerebral hemorrhage and 120 patients with hypertension admitted to the first Department of Neurology,Tianshui First People's Hospital from January 2018 to October 2019 were collected and patients with hypertensive cerebral hemorrhage were divided into three groups according to hemorrhage amount with 60 cases in less than 10 mL,35 patients in the group of more than or equal to 10 mL and less than or equal to 30 mL,20 patients in the group of more than 30 mL and less than 60 mL.Homocysteine and uric acid measurement were performed on all the included patients within 24 hours after admitted to hospital.The changes of homocysteine and uric acid between hypertensive cerebral hemorrhage patients and hypertensive patients were compared,and their effects on the pathogenesis of hypertensive cerebral hemorrhage were evaluated.Results The changes of homocysteine and uric acid in hypertensive cerebral hemorrhage patients and hypertensive patients were significantly different,with statistical significance(P<0.05).The increase of homocysteine and uric acid was mostly seen in the patients with small and medium hemorrhage amount rather than the patients with large amount of hemorrhage,indicating that although homocysteine and uric acid were highly sensitive,their data changes had little relationship with the amount of cerebral hemorrhage.Conclusion Homocysteine and uric acid are high risk factors for hypertensive cerebral hemorrhage.Routine monitoring of homocysteine and uric acid for hypertensive patients and appropriate intervention can reduce the risk of cerebral hemorrhage.
