摘　　要：目的分析新辅助放化疗后行腹腔镜下低位直肠癌保肛术的临床效果。方法选取2016年1月~2019年12月60例低位直肠癌患者,采用随机分组法,将研究对象划分A组(n=30)和B组(n=30),两组均实施新辅助放化疗,A组行开腹手术,B组行腹腔镜下保肛术,比较两组手术指标、保肛率、并发症发生率。结果两组患者手术时间差异不显著(P>0.05);B组患者术中出血量少于A组;B组患者保肛率高于A组;B组患者切口感染、肠梗阻、尿潴留、吻合口瘘等并发症发生率低于A组,两组之间数据比较差异有统计学意义(P<0.05)。结论在低位直肠癌患者中实施新辅助放化疗联合腹腔镜下保肛术,术中出血量少,保肛率高,很少出现并发症。Objective To analyze the clinical effect of laparoscopic anus-preserving surgery for low rectal cancer after neoadjuvant radiotherapy and chemotherapy.Methods 60 patients with low rectal cancer from January 2016 to December 2019 were selected as the subjects of research and divided into the group A(n=30)and the group B(n=30)through random grouping method.With neoadjuvant radiotherapy and chemotherapy being given to both groups,the group A underwent a laparotomy and the group B underwent laparoscopic anus-preserving surgery.The surgical indexes,the anus-preserving rate and the incidence rate of complications were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05).The intraoperative blood loss of the patients in group B was less than that in group A.The anus-preserving rate of the patients in group B was higher than that in group A.The incidence rate of complications such as incision infection,intestinal obstruction,urinary retention and anastomotic fistula,etc.of the patients in group B was lower than that in group A,and the differences between the two groups were statistically significant(P<0.05).Conclusion The application of neoadjuvant radiotherapy and chemotherapy combined with laparoscopic anus-preserving surgery to the patients with low rectal cancer has the advantages of less blood loss,higher anus-preserving rate and fewer complications.
