摘　　要：目的探讨运用IsoFlux^TM系统在乳腺癌患者外周血循环肿瘤细胞(Circulating tumor cells,CTCs)检测中的临床应用价值。方法回顾性分析深圳市人民医院甲乳外科2015年12月~2017年5月收治的23例女性乳腺癌患者运用IsoFlux^TM系统进行循环肿瘤细胞检测的临床资料,并将CTCs数量与肿块大小、TNM分期的关系进行统计学分析。结果23例乳腺癌患者中,可手术乳腺癌患者22例,术前使用IsoFlux^TM系统全部检测到CTCs,阳性捕获率100%,每7.5 mL全血样本中CTCs数目2~4201个不等,中位数173.5个;另1例为乳腺癌术后2年出现双肺多发结节、怀疑肺转移的患者,未检测到CTCs,根据CTCs结果对其肺部结节初步排除肺转移可能。22例可手术乳腺癌患者的肿块大小和log(CTCs)显著相关,CTCs的数量与乳腺癌TNM分期(Ⅰ、Ⅱ、Ⅲ)不存在显著的相关性。结论此次将IsoFlux^TM系统应用于乳腺癌CTCs检测的探索性研究初步证明,该系统敏感性高、捕获力强,是乳腺癌患者CTCs检测的一个理想选择,有助于监测乳腺癌术后的复发和转移情况。Objective To explore the clinical application value of IsoFlux^TM system in the detection of circulating tumor cells(CTCs)in breast cancer patients.Methods The clinical data of 23 female breast cancer patients whose circulating tumor cells was detected using the IsoFlux^TM system in the Department of Thyroid and Breast Surgery,Shenzhen People's Hospital from December 2015 to May 2017 were retrospectively analyzed.The relationship between the number of CTCs,tumor size,and TNM stage was statistically analyzed.Results Among the 23 breast cancer patients,22 were operable breast cancer patients.CTCs were all detected by IsoFlux^TM system before operation.The positive capture rate was 100%.The number of CTCs in each 7.5 mL whole blood sample ranged from 2 to 4201,with the median of 173.5.Another case was a patient with multiple lung nodules and suspected lung metastases 2 years after breast cancer surgery.CTCs was not detected.Based on the CTCs results,we initially ruled out the possibility of lung metastases for lung nodules.The size of lump in 22 patients with operable breast cancer was significantly correlated with log(CTCs).There was no significant correlation between the number of CTCs and the TNM stages(Ⅰ,Ⅱ,Ⅲ)of breast cancer.Conclusion The exploratory study of applying the IsoFlux^TM system to the detection of breast cancer CTCs initially proved that the system has high sensitivity and strong capture power,and is an ideal choice for CTCs detection of breast cancer patients,which helps to monitor the recurrence and metastases of breast cancer after surgery.
