M型超声观测颈动脉血管同步性与缺血性脑卒中ROC曲线分析研究

摘  要:目的ROC曲线分析应用M型超声观测颈动脉血管管壁运动同步性预测缺血性脑卒中的价值。方法研究我院经CT检查确诊单侧缺血性脑卒中组患者289例,对照组经CT检查无缺血性脑卒中患者256例。应用M型超声于卒中同侧颈动脉相同节段分别测量颈总动脉、颈内动脉血管前壁、后壁管壁峰值搏动幅度(PW)、达峰时间(PT)、心电图R峰与管壁峰值搏幅时间差(RT)。计算相同节段颈动脉前、后壁运动幅度差(SPW)、运动峰值时间差(SRT)、达峰时间差(SPT)。应用ROC曲线分析各参数对缺血性脑卒中的预测价值。结果两组间颈总动脉SPW(P=0.003)、SRT(P=0.007)、SPT(P=0.032)差异有统计学意义。两组间颈内动脉SPW(P=0.001)、SRT(P=0.001)、SPT(P=0.003)差异有统计学意义。颈总动脉SPW、SRT、SPT预测缺血性脑卒中AUC分别为:0.908、0.901、0.885,灵敏度分别为:85.6%、80.4%、90.9%,特异度分别为:89.9%、88.4%、74.1%。颈内动脉SPW、SRT、SPT预测缺血性脑卒中AUC分别为:0.682、0.801、0.885,灵敏度分别为:51.8%、67.0%、55.4%,特异度分别为:88.2%、86.3%、81.6%。结论缺血性脑卒中组与对照组间颈动脉血管运动时间同步性和运动幅度同步性与缺血性脑卒中发病率存在着相关性。M型超声测量颈动脉SPW、SPT、SRT对缺血性脑卒中有较好的临床预测价值。有望在基层机构推广和应用M型超声观测颈动脉功能尽早预测缺血性脑卒中等终点事件的发生。Objective To analyze the application value of M-mode ultrasound in observation of carotid artery vessel wall motion and prediction of ischemia stroke with ROC curve.Methods 289 cases who were diagnosed with single side ischemia stroke by CT examination(stroke group),and 256 cases group without ischemia stroke by CT examination(Control group)were scleted.Use the M-mode ultrasound was used to observe the synchrony of the common carotid artery and internal carotid artery wall at the same segment of the ipsilateral carotid artery.Anterior and posterior walls of the common carotid artery and internal carotid artery′s subtract wall peak dynamic range(PW),peak time(PT),and the time difference between ECG R peak and wall peak dynamic range(R-time,RT)were measured.The subtract wall peak dynamic range(SPW),the subtract R-Time(SRT),and the subtract peak time(SPT)between the anterior and posterior walls of the same segment were calculated.The statistical methods were used to compare the differences between SPW,SRT and SPT between the two groups at the same segment of the vascular.ROC curve was used to analyze the predictive value of each parameter in ischemia stroke.Results At the common carotid artery,there were statistically significant difference in SPW(P=0.003),SRT(P=0.007)and SPT(P=0.032)between the two groups.At the internal carotid artery,there were statistically significant difference in SPW(P=0.001),SRT(P=0.001)and SPT(P=0.003)between the two groups.The ROC curve analyze results in common carotid artery SPW,SRT and SPT to prediction ischemia stroke AUC were:0.908,0.901,0.885,sensibility were:85.6%,80.4%,90.9%,and specificity were:89.9%,88.4%,74.1%,The internal carotid artery AUC were:0.682,0.801,0.885,sensibility were:51.8%,67.0%,55.4%,and specificity were:88.2%,86.3%,81.6%.Conclusions There is a correlation at the time synchronization and amplitude synchronization of carotid artery motion between the incidence of ischemic stroke between the two group.The measurement the carotid′s SPW,SPT,SRT by M-mode ultrasoun
