血清microRNA-497定量检测与结直肠癌血管生成的关系及其临床意义

摘　　要：目的定量检测结直肠癌(colorectal cancer,CRC)患者血清microRNA-497(miR-497)表达量,探讨其与结直肠癌血管生成的关系及其临床意义。方法采用实时荧光定量RT-PCR检测93例CRC患者术前、30例健康者血清miR-497表达量;以ELSIA法检测血清VEGF、血管生成素-2(Angiopoietin-2,Ang-2)含量;以免疫组化法检测93例CRC癌组织及30例正常结直肠黏膜组织中CD34表达,计算肿瘤微血管密度(microvessel density,MVD);术后随访3年;分析术前血清miR-497表达量与CRC血管生成、病理特征及预后的关系。结果CRC组血清miR-497表达量显著低于正常对照组(P<0.01),血清VEGF、Ang-2含量及癌组织中MVD显著高于正常对照组(P<0.01);miR-497低表达组血清VEGF、Ang-2含量及MVD均显著高于miR-497高表达组(P<0.01);CRC组血清miR-497表达量与血清VEGF、Ang-2含量及MVD均呈负相关(r=-0.437、-0.461、-0.383,P<0.01);术前血清miR-497表达量与CRC分化程度、TNM分期、是否淋巴结转移及是否远处转移呈负相关(P<0.01),与术后复发转移呈负相关(P<0.01)、与生存期长短呈正相关(P<0.01)。Cox多因素分析显示术前血清miR-497表达量是影响CRC患者术后预后的独立高危因素。结论血清miR-497表达量降低可促进CRC新生血管生成及侵袭转移,与CRC患者不良预后相关;术前血清miR-497表达量可成为CRC患者预后评价指标。Objective To investigate the correlation between quantitative detection of microRNA-497(miR-497)in serum and angiogenesis in colorectal cancer(CRC)and its clinical significance.MethodsThe quantitative expression of miR-497 in serum were detected by real-time fluorescence quantitative RT-PCR,the VEGF and Ang-2 in serum were detected by ELISA,The expression of CD34 were detected by immunohistochemistry in CRC(n=93)and normal control(n=30).The MVD were calculated.The 3 years postoperative prognosis was followed-up.The relationship between the preoperative miR-497 expression levels in serum in the CRC and angiogenesis,the pathologic features and prognosis were analyzed.ResultsThe serum miR-497 expression levels in CRC group were significantly lower than those in the normal control group(P<0.01),the VEGF and Ang-2 serum levels and MVD in CRC tissues were significantly higher than those in the normal control group(P<0.01).The VEGF and Ang-2 serum expression levels and MVD in low miR-497 expression group were significantly higher than those in high miR-497 expression group.The serum miR-497 expression levels were negatively related with the serum VEGF and Ang-2 expression levels and MVD in CRC group(r=-0.437,-0.461,-0.383,P<0.01).The preoperative serum miR-497 expression levels in CRC group were negatively related with the differentiated degree of tumor,TNM stage,lymph node metastasis,distant metastasis and postoperative recurrence and metastasis(P<0.01),and were positively related to the postoperative survival(P<0.01).Multivariate Cox regression analysis revealed that preoperative serum miR-497 expression levels were independent prognostic factor for CRC.Conclusion The low serum miR-497 expression levels in CRC promote tumor angiogenesis and invasion and metastasis,were related with poorprognosis,and can be a prognostic indicator of CRC.
