天麻钩藤饮加减方治疗脑梗死伴高血压肝阳上亢证临床研究

摘　　要：目的:观察天麻钩藤饮加减方治疗脑梗死伴高血压肝阳上亢证的临床疗效及对预后的影响。方法:将92例脑梗死伴高血压肝阳上亢证患者随机均分为研究组(天麻钩藤饮加减方联合常规西药治疗)和对照组(常规西药治疗)。比较两组治疗效果,比较两组治疗前及治疗4周后中医症候积分、神经功能[美国国立卫生院卒中量表(NIHSS)]、血压[收缩压(SBP)、舒张压(DBP)]、疾病相关因子[同型半胱氨酸(Hcy)、血管性假血友病因子(vWF)]水平。结果:治疗4w后,两组治疗有效率比较差异无统计学意义(P>0.05)。治疗4w后,两组患者口舌歪斜、偏身麻木、半身不遂、心悸便溏、手足肿胀症候积分均较治疗前显著下降,研究组明显低于对照组;两组患者NIHSS、血压(SBP、DBP)、疾病相关因子(Hcy、vWF)水平均较治疗前显著下降,研究组明显低于对照组(P<0.05)。结论:天麻钩藤饮加减方治疗脑梗死伴高血压肝阳上亢证疗效显著,能够有效改善患者症状,降低神经功能缺损程度及血压。Objective:To study the effect of Modified Tianma Gouteng Decoction on clinical efficacy and prognosis in the treatment of patients with cerebral infarction complicated with hypertension of liver-Yang hyperactivity syndrome.Methods:Ninety-two patients with cerebral infarction complicated with hypertension of liver-Yang hyperactivity syndrome were randomly and evenly divided into a study group(Modified Tianma Gouteng Decoction combined with conventional western medicine treatment) and a controlled group(conventional western medicine treatment).The treatment effects were compared between the two groups,and the TCM symptoms scores,neurological function [National Institutes of Health Stroke Scale(NIHSS)],blood pressure [systolic blood pressure(SBP),diastolic blood pressure(DBP)] and disease-related factors [homocysteine(Hcy),von Willebrand factor(vWF)] were compared between the two groups before treatment and after 4 weeks of treatment.Results:After 4 weeks of treatment,there was no significant difference in the effective rate of treatment between the two groups(P>0.05).After 4 weeks of treatment,the scores of symptoms of deviation of the eye and mouth,hemianesthesia,hemiplegia,palpitations and loose stool and swollen hands and feet in the two groups were significantly decreased compared with those before treatment,and the scores in study group were significantly lower than those in control group.The NIHSS score,blood pressure(SBP,DBP) and levels of disease-related factors(Hcy,vWF) in the two groups were significantly decreased compared with those before treatment,and the indexes in the study group were significantly lower than those in the controlled group(P<0.05).Conclusion:Modified Tianma Gouteng Decoction has significant efficacy in the treatment of cerebral infarction with hypertension of liver-Yang hyperactivity syndrome,and it can effectively improve the symptoms,reduce the neurological deficit degree and blood pressure.
