甲下外生性骨疣12例临床分析

摘　　要：2014-2019年间我院皮肤科收治的12例甲下外生性骨疣患者中男5例,女7例,发病年龄5~69岁,平均28.83岁。发病部位为足趾者9例,手指者3例。12例患者均采用手术切除治疗,随访3~12个月均无复发。典型临床表现为指(趾)甲下或远端单发外生性结节肿块,质硬,部分患者伴有疼痛,同时伴有不同程度的指(趾)甲变形或缺损。X线检查示指(趾)骨内外侧或远端末端骨性增生物。组织病理检查示真皮内类似骨刺的成熟骨结构,分为4层(由上往下依次为):纤维层(相当于骨膜),增生活跃的纤维母细胞层,纤维软骨层,软骨化骨区域。The clinical data of 12 cases with subungual exostosis were retrospectively analyzed.Of 12 cases,5 were male and 7 were female,the age ranged from 5 to 69 years old with median age of 28.83.The lesions located on the toes in 9 cases and on fingers in 3 cases.All cases were treated with surgical excision.There was no recurrence after 3 months to 1 year follow-up.Subungual exostosis usually manifested as a hard,subungual nodule that elevated the nail plate,some with pain and deformed or defected nail plates.Radiograph of the digit demonstrated the subungual bony proliferation.Histologically,the cartilaginous cap of exostoses was made of fibrocartilage,composed of 4 layers(from the outer surface to the inner base):fibrous zone representing periosteum,active fibroblast proliferation zone,fibrocartilage zone,trabecular bone formation zone.
