突发性聋的耳鸣发生率及与听力恢复的同步性观察

摘　　要：目的探讨突发性聋所伴随的耳鸣与内耳病变之间的相关性。方法对2011年9月~2019年9月于广州中医药大学第一附属医院耳鼻喉科住院的824例突发性聋患者进行回顾性分析,统计耳鸣的发生率、耳鸣对听力恢复的影响及耳鸣与听力恢复的同步性。结果①824例突发性聋患者中,耳鸣的发生率为89.9%。②有耳鸣与无耳鸣两组听力痊愈率及总有效率无显著性差异(P>0.05)。③在127例伴有耳鸣且听力完全恢复正常的患者中,耳鸣消失者仅占41.7%,耳鸣仍存在者占58.3%;而在117例耳鸣消失的患者中,听力恢复正常者仅占45.3%,听力未恢复正常者占54.7%。结论并非所有的突发性聋都会伴有耳鸣,耳鸣是否出现与听力是否恢复无关,超过一半的患者耳鸣与听力恢复不同步,因此耳鸣与内耳病变之间的关系值得进一步研究。Objective To investigate the correlation between tinnitus and inner ear lesions associated with sudden deafness.Methods A retrospective study on 824 sudden deafness patients who admitted to The First Affiliated Hospital of Guangzhou University of Chinese Medicine was conducted from September 2011 were to September 2019.The incidence of tinnitus,the effect of tinnitus on hearing recovery,the synchronization of tinnitus and hearing recovery were investigated.Results Among 824 patients with sudden deafness,the incidence of tinnitus was 89.9%.There was no significant difference between hearing recovery rate and total effective rate between the two groups with and without tinnitus(P>0.05).Among 127 cases with tinnitus and complete hearing recovery,41.7%had their tinnitus disappeared while 58.3%remained.Of 117 patients who’s tinnitus disappeared,only 45.3%had hearing recovery while 54.7%had not.Conclusion Not all sudden deafness patients were accompanied by tinnitus.The presence of tinnitus has nothing to do with hearing recovery.Tinnitus disappearance and hearing recovery did not happen simultaneously in more than half of patients,so the relationship between tinnitus and inner ear lesions is worthy of further study.
