摘  要:目的:观察颈外静脉置管大鼠相关性血栓形成和血清一氧化氮(NO)、组织因子(TF)水平变化及其相关性。方法:雄性SD大鼠120只,随机均分为空白组、手术组、置管组。空白组不作任何手术处理;手术组暴露右颈外静脉后,即刻缝合手术切口;置管组右颈外静脉切开,置入导管。分别于术后第1天、4天、7天、10天、14天随机取各组大鼠8只,截取右颈外静脉进行病理切片,观察血栓形成情况,同时从腹主动脉采血分离血清,采用硝酸还原酶法测定NO浓度,ELISA测定TF含量,比较各组各指标水平差异,并分析其与血栓形成的关系。结果:空白组及手术组均无血栓形成。置管组大鼠85%有血栓形成(34/40)。置管组术后各时点血清NO水平较空白组降低(P<0.01),术后第10天、14天,血清NO水平较手术组降低(P<0.01);置管组术后第1天、4天、7天、10天血清TF水平较空白组及手术组升高(P<0.01)。置管组术后第1天-10天,血清NO含量逐渐降低,而TF含量逐渐增加(P<0.01)。相关分析显示置管组术后第1天-14天,大鼠血清NO与其血栓形成率呈负相关(r=-0.607),术后第1天-10天,血清TF与血栓形成率呈正相关(r=0.438)。结论:静脉置管可致血栓形成,NO降低、TF升高可能与此有关。Objective: To observe the relationship between thrombosis and the changes of serum nitric oxide(NO) and tissue factor(TF) in rats with external jugular vein catheterization. Method: 120 male SD rats were randomly divided into blank group, operation group and catheterization group. Blank group: no surgical treatment. Operation group:no vein incision was performed, only the right external jugular vein was exposed, and the surgical incision was immediately sutured.Catheterization group: after exposing the right external jugular vein, the vein was incised and a catheter was inserted. Eight rats in each group were randomly selected on the 1 st, 4 th, 7 th, 10 th and 14 th day after operation to cut off the right external jugular vein for pathological section observation of thrombosis. Meanwhile, blood was collected from the abdominal aorta to separate serum, the concent of NO was measured by nitrate reductase method, the concent of TF was measured by ELISA, the difference of each index level in each group was statistically analyzed, and its relationship with thrombosis was analyzed. Results: There was no thrombosis in the blank group and the operation group.In the catheterization group,34 rats had thrombosis,and the incidence was 85%.The level of NO in the catheterization group was lower than that in the blank group at each time point after surgery(P<0.01),and the level of NO was lower than that in the operation group on the 10 th and 14 th day after operation(P<0.01);the level of TF in the catheterization group was higher than that in the blank group and the operation group on the 1 st,4 th,7 th and 10 th day after operation(P<0.01).In the catheterization group,the content of serum NO decreased gradually,while the content of TF increased gradually on the 1 st-10 th day(P<0.01).Correlation analysis showed that the serum NO of the rats in the catheterization group was negatively correlated with the thrombosis rate on the 1 st-14 th day(r=-0.607),and the serum TF was positively correlated with the thrombosis rate on the
