摘　　要：目的探讨CT仿真内窥镜成像(CTVE)联合血清肿瘤标志物检测对胃癌的诊断价值。方法选取62例胃癌患者和58例胃部良性病变患者,所有患者均接受了CTVE及血清肿瘤标志物检测,比较胃癌患者和胃部良性病变患者的血清肿瘤标志物水平,并以病理学检查结果为金标准,计算CTVE联合血清肿瘤标志物对胃癌的鉴别诊断价值。结果胃癌患者血清中的癌胚抗原(CEA)、糖类抗原724(CA724)水平均明显高于胃部良性病变患者,差异均有统计学意义(P﹤0.01)。经CTVE检查,120例胃部病变患者中,胃癌65例,胃部良性病变55例;其中,漏诊胃癌7例,误诊胃癌10例。CEA、CA724及CTVE三者联合检测诊断胃癌的灵敏度为96.77%,特异度为89.66%,阳性预测值为90.91%,阴性预测值为96.30%,约登指数为0.86。结论胃癌患者血清中的CEA、CA724水平较高,CEA、CA724及CTVE联合检测可以提高对胃癌诊断的灵敏度和特异度。Objective To evaluate the clinical value of computed tomography virtual endoscopy(CTVE)combined serum tumor markers in the diagnosis of gastric cancer.Method 62 patients with gastric cancer and 58 patients with gastric benign lesions were selected and received CTVE and assay of serum tumor markers.The levels of serum tumor markers in patients with gastric cancer and patients with gastric benign lesions was compared,and the diagnostic value of CTVE combined serum tumor markers in gastric cancer was calculated using pathological examination as the gold mark.Result The levels of serum carcinoembryonic antigen(CEA),carbohydrate antigen 724(CA724)in gastric cancer patients were significantly higher than those in gastric benign lesions patients(P<0.01).CTVE examination showed 65 cases of gastric cancer,55 cases of benign gastric lesions,7 cases of under diagnosis of gastric cancer,and 10 cases of misdiagnosis of gastric cancer.The sensitivity of CEA,CA724 and CTVE combined detection in the diagnosis of gastric cancer was 96.77%,the specificity 89.66%,the positive predictive value 90.91%,the negative predictive value 96.30%,and the Yoden index 0.86.Conclusion The levels of serum CEA,CA724 in gastric cancer patients are higher and combined detection of CEA,CA724 and CTVE can significantly improve the sensitivity and specificity of diagnosis.
