摘　　要：目的分析我国主诊医师负责制的现状、应用效果及问题,为完善和推广该制度提供实施建议。方法通过CNKI数据库跨库检索从1979年1月1日至2019年10月13日关于主诊医师负责制的中文文献,对其量化分析。结果纳入研究的中文文献132篇,以期刊为主,大多来自经济发达的京沪及江浙地区;主要内容涉及实施主诊医师负责制的效果评价、存在的问题、实施对策、绩效考核及管理模式等,其中效果评价的文献共有53篇,基本上从工作效率、医疗安全、质量、服务等方面对该制度给予了正面评价;有29篇文献阐述了实施该制度现存的问题,主要为科主任行政权力减弱影响科室团队协作;有21篇文献针对问题给出了建议。结论主诊医师负责制对提高医疗安全、质量、效率,改善医疗服务等有较好的促进作用。强化科主任管理职能、落实质量管理核心制度、合理分配医疗资源及调整绩效考核模式是完善该制度的有效措施。Objective To analyze the current situation, effectiveness and problems of the attending in charge in China, and provide suggestions on improving and promoting the system. Methods The Chinese literature on attending physician responsibility system from January 1, 1979 to October 13, 2019 was searched through CNKI database, and its quantitative analysis was made.Results 132 literatures in Chinese are studied. Most of them are journal papers, and from developed areas, such as Beijing, Shanghai, Zhenjiang and Jiangsu. They mainly focus on the effectiveness evaluation, known issues, methods of execution, performance appraisal, administration mode of attending in charge. 53 literatures show positive on effectiveness, especially on improving medical security, quality of service. 29 literatures concern on the issues, which manifest as weakening the collaboration of a department if the director exercises too much administrative operations. But 21 of them give suggestions on how to resolve the issues.Conclusion Attending in charge is able to improve medical safety, quality, efficiency, service, and etc. Strengthening the management function of department directors, implementing the core system of quality management, rational allocation of medical resources and adjusting the performance evaluation model are effective measures to improve the system.
