摘　　要：目的应用Logistic回归分析探讨并筛选影响经腹超声胰腺可视化质量的人体参数,以利于胰腺检查手段的临床决策。方法采用横断面研究,选取门诊患者100例,记录其性别、年龄、体重指数(BMI)、腰围、腹壁厚度,观察其胰头、胰体、胰尾经腹超声检查的可视化质量并分级。进行多因素回归分析,建立Logistic回归模型及ROC曲线,获得胰腺经腹超声可视化质量的影响因素及截断值。结果经单因素分析胰头经腹超声可视化质量受BMI、腰围、腹壁厚度影响,差异均有统计学意义(P<0.05);胰体受BMI、腰围、腹壁厚度影响,差异有统计学意义(P<0.05);胰尾受年龄、BMI、腰围、腹壁厚度影响,差异有统计学意义(P<0.05)。经Logistic多因素回归分析胰头(OR=1.145,P=0.002)、胰体(OR=1.136,P=0.013)经腹超声可视化质量受腰围的影响;胰尾经腹超声可视化质量受腰围(OR=1.117,P=0.022)和腹壁厚度(OR=4.527,P=0.012)的影响。应用ROC曲线分析得到影响胰头可视化质量的腰围Cutpoint值为91 cm;影响胰体可视化质量的腰围Cutpoint值为90 cm;影响胰尾可视化质量的腰围Cutpoint值为85 cm,腹壁厚度Cutpoint值为2.9 cm。结论腰围是预测经腹超声可视化质量的有意义的指标,根据研究所得到的腰围Cutpoint值可以成为临床医生选择胰腺影像学检查的根据。Objective To investigate and screen the visualization body parameters influencing the visual quality of transabdominal ultrasound of pancreas for better pancreas imaging examination.Methods A total of 100 outpatients who underwent transabdominal pancreas ultrasound examination in our hospital were enrolled in the cross-sectional study.The patient’s age and sex,body mass index(BMI),waist circumference and abdominal wall thickness were recorded.The visual quality of the pancreas was observed and graded.Moreover,multiple regression analysis was performed to establish Logistic regression model and ROC curve to obtain factors and cutpoint value influencing the visual quality of transabdominal ultrasound of pancreas.Results The single factor analysis showed the visual quality of pancreatic head was influenced by BMI(P<0.01),waist circumference(P<0.001)and abdominal wall thickness(P<0.01),pancreatic body was influenced by BMI(P<0.01),waist circumference(P<0.01)and abdominal wall thickness(P<0.05)and pancreatic tail was influenced by age(P<0.05),BMI(P<0.01),waist circumference(P<0.01)and abdominal wall thickness(P<0.01 or P<0.05).Moreover,multiple regression analysis showed that the visual quality of the pancreatic head(P<0.01)and pancreatic body(P<0.05)were influenced by the waist circumference(P<0.05)and the visual quality of the pancreatic tail was influenced by the waist circumstances(P<0.05)and abdominal wall thickness(P<0.05).In addition,ROC curve analysis showed that the waist circumference cutpoint value influencing the visual quality of pancreatic head,pancreatic body,pancreatic tail and abdominal wall thickness were 91cm,90cm,85cm and 2.9cm,respectively.Conclusion Waist circumference is a significant index to predict the quality of transabdominal ultrasound visualization.The Cutpoint value of waist circumference can be used in clinical practice to select pancreatic imaging examination.
