摘　　要：后巩膜加固术(PSR)是防治高度近视的重要方法,该手术通过从后路增强后巩膜组织,进而防止甚至减轻由于病理性近视眼轴进行性延伸所引起的眼底后极部组织损害。关国华教授在中医"治未病"思想的指导下,提出PSR联合中药防治高度近视,尤其是治疗发展迅速的青少年高度近视的诊疗思路,并与现代诊疗手段相结合,如光学相干断层扫描血管成像(OCTA)、微创玻璃体视网膜手术、抗血管内皮生长因子(VEGF)治疗等,临床疗效更加理想。同时主张术前、术后均使用中医辨证治疗,通过眼局部辨证,将治疗高度近视分为健脾益气和滋养肝肾2个阶段。本文将对其经验进行总结介绍。Posterior scleral reinforcement is an important procedure for the prevention and treatment of high myopia. This operation prevents or even reduces tissue damage in the posterior pole of the fundus caused by the progressive extension of the pathological myopia by enhancing the posterior scleral tissue from the posterior approach. Under the guidance of the idea of preventive treatment of diseases, Professor Guan proposed the idea of posterior scleral reinforcement com-bined with Traditional Chinese Medicine to prevent and treat high myopia, especially the rapidly developing high myopia cases of adolescents, and combined with modern methods of diagnosis and treatment, such as optical coherence tomography angiography(OCTA), minimally invasive vitreo-retinal surgery, anti-vascular endothelial growth factor(VEGF) therapy, etc. The clinical efficacy was more favorable. Professor Guan advocated that Traditional Chinese Medicine should be used before and after surgery. According to focal syndrome differentiation, the high myopia was divided into two stages of fortifying the spleen and invigorating qi and nourishing the liver and kidney. This article summarized Professor Guan’s experience.
