从“气机升降”浅析非动脉炎性前部缺血性视神经病变

摘　　要：气机升降理论是中医基础理论的重要组成部分之一,气机升降出入功能正常是人体生理活动得以实现的基础。眼居头面,为九窍之一,其生理功能的正常发挥有赖于清阳之气的上承,因此,眼科疾病与人体气机升降功能失司密不可分。非动脉炎性前部缺血性视神经病变(NA-AION)是临床上较为常见的、好发于中老年人的视神经疾病,属中医眼科"暴盲"范畴。本文从"气机升降"出发,辨析NA-AION的病因病机,认为此病与气机郁滞、升降失司关系密切。气机失于调畅,升降出入失司,目失精微物质充养而致病。依据本病的病因病机提出调气机、复升降、开玄府的治疗思路,并结合典型病例陈述诊疗体会。The qi movement is one of the important components of the basic theory of Traditional Chinese Medicine, and that is the basis for the normal physiological activities of the human body. Eyes are one of the nine orifices on the face, and its normal working of physiological func-tions depends on the upward moment of qingyang qi, so the relationship between ophthalmological diseases and the loss of the function of the human qi movement is inseparable. Non-arteritic anterior ischemic optic neuropathy(NA-AION) is a clinically common optic nerve disease that is more common in the elderly. The NA-AION belongs to the category of sudden blindness in Chinese ophthalmology. This article is based on qi movement, to analyze the TCM etiology and pathogenesis of NA-AION, and it is believed that this disease is closely related to qi movement. The disharmony of qi movement, uncontrolled upward, downward, inward and outward movement, malnutrition of eyes lead to this condition. According to the etiology and pathogenesis of the disease, the treatment ideas of regulating the qi movement, resetting upward and downward movement, and opening mysterious mansion were proposed, and the diagnosis and treatment experience was combined with typical cases.
