精神分裂症及其遗传高危人群血浆炎性细胞因子水平与临床症状的相关研究

摘　　要：目的探究精神分裂症和精神分裂症遗传高危人群的血浆炎性细胞因子水平及其在精神分裂症发生发展中的作用。方法纳入289名受试者,精神分裂症患者组94例、精神分裂症遗传高危组60例和健康对照组135例,采用Luminex多因子检测法测定受试者炎性细胞因子IL-6、TNF-α和IL-1β浓度,采用17项汉密尔顿抑郁量表(HAMD-17)、汉密尔顿焦虑量表(HAMA)和简明精神病评定量表(BPRS)评定受试者的抑郁、焦虑症状以及精神症状严重程度。结果患者组IL-6高于高危组和对照组(P<0.05);患者组和高危组TNF-α水平高于对照组(P<0.05);关联分析显示,患者组IL-6水平与HAMD、HAMA评分均呈正相关,在高危组IL-6水平与HAMD评分呈正相关(P<0.05);患者组TNF-α水平与HAMA、BPRS评分均呈正相关(P<0.05)。结论精神分裂症及精神分裂症遗传高危人群均存在炎性细胞因子激活,且炎性细胞因子水平的变化可能与精神分裂症临床症状有关。Objective To investigate the changes of plasma inflammatory Cytokines levels in genetic high-risk of schizophrenia(GHR-SZ)and schizophrenia(SZ),and to evaluate the roles in the occurrence and development of SZ. Methods A total of 289 subjects,94 SZ patients,60 GHR-SZ individuals and 135 healthy controls(HC)were enrolled in this study. The levels of interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α)and interleukin-1β(IL-1β)in plasma were measured by Luminex assay. The severity of depression,anxiety symptoms and mental symptoms were evaluated by Hamilton Depression Scale-17、Hamilton Anxiety Scale and Brief Psychiatric Rating Scale. Results The level of IL-6 in SZ was significantly higher than that in GHR-SZ and HCs(P<0.05). The level of TNF-α in SZ and GHR-SZ groups was higher than that in HCs(P<0.05). In SZ,IL-6 was positively correlated with HAMD and HAMA score. In GHR-SZ,IL-6 was positively correlated with HAMD total score(P<0.05). In SZ group,TNF-α level was positively correlated with HAMA and BPRS score(P<0.05). Conclusion The activation of plasma inflammatory cytokines exists in both SZ and GHR-SZ,which may be related to clinical symptoms of SZ.
