摘  要:目的研究在不同宫颈组织中hTERC基因的表达情况及其与高危型HPV阳性的相关性,探讨其在宫颈病变中的诊断价值。方法选取昆明医科大学第二附属医院就诊的1200例宫颈癌筛查患者,分别用HC2、SPR法和FISH技术获取HPV感染和hTERC基因扩增情况,任何具有阳性结果的患者均通过组织活检作为确诊金标准。根据病理活检结果进行分组:正常组32例,CINI38例,CINII-CINIII66例,宫颈癌14例。结果hTERC基因在各宫颈病变分组中的表达率分别为12.5%、34.21%、53.03%、85.71%,hTERC基因表达率在各病理级别间的差异有统计学意义(χ^2=28.90,P=0.000);随着宫颈病变的进展,HC2、SPR技术检测HPV感染率逐渐升高,hTERC基因表达也随着增长,且与二者呈正相关(r1=0.538,P 1=0.000;r2=0.796,P2=0.000)。结论hTERC基因异常扩增和HPV感染率随组织病变升级而升高,二者联合检测对判断早期宫颈病变有指示作用。Objective To study the expression of hTERC gene in different cervical tissues and its correlation with high-risk HPV positive,and to explore its diagnostic value in cervical lesions.Methods A total of 1200 cervical cancer screening patients admitted to the Second Affiliated Hospital of Kunming Medical University were selected.HC2,SPR and FISH were used to obtain HPV infection and hTERC gene amplification,respectively.Any patients with positive results was diagnosed by biopsy and divided into different groups(32 in the normal group,38 in CINI group,66 in CINII-CINIII group and 14 in the cervical cancer group).Results According to the pathological biopsy result,the expression rates of hTERC gene in each cervical lesion group were 12.5%, 34.21%, 53.03% and 85.71%, respectively. The differences in the expression rates of hTERC gene in each pathological grade were statistically significant (χ^2= 28.90,P = 0.000). With the development of cervical lesions, the infection rate of HPV detected by HC2 and SPR increased gradually, and the expression of hTERC gene also increased with the increase, and was positively correlated with both (r1=0.538, P1=0.000;r2 = 0.796, P2 = 0.000). Conclusion Abnormal amplification of hTERC gene and HPV infection rate increases with the escalation of tissue lesions. The combined detection of hTERC gene and HPV infection rate is indicative of early cervical lesions.
