探讨早期青光眼适合行激光周边虹膜切除术的虹膜曲度

摘　　要：目的:探讨早期原发性闭角型青光眼(PACG)适合施行激光周边虹膜切除术(LPI)的虹膜曲度范围。方法:回顾性分析2015年至2018年滕州市中心人民医院的早期PACG 539例(920眼)的临床资料,患者均行LPI。比较术前及术后1周眼压和前房角开放距离(AOD 500)的差异。结果:LPI术后有610眼眼压下降至≤21 mmHg(1 mmHg=0.133 kPa),术后AOD 500明显增加(P=0.014),其术前虹膜曲度值均≥0.210 mm;有253眼眼压仍>21 mmHg,术后AOD 500小幅度增加(P=0.083),其术前虹膜曲度为0.197~0.209 mm;另57眼眼压无明显下降,AOD 500无明显变化(P=0.558),其术前虹膜曲度为0.076~0.196 mm。结论:术前虹膜曲度≥0.210 mm的早期PACG适合行LPI,术后AOD 500增加明显,降压效果显著。术前虹膜曲度越高,其术后AOD 500增加越明显,而且降眼压效果越好。Objective To investigate the range of iris curvature in early primary angle-closure glaucoma(PACG)suitable for laser peripheral iridectomy(LPI).Methods This was a retrospective study.The clinical data with 920 eyes of 539 cases with early PACG from 2015 to 2018 in Tengzhou Central People’s Hospital were collected.All cases received LPI.The difference in intraocular pressure and anterior chamber angle(AOD500)were compared before and 1 week after surgery.Results After LPI,the intraocular pressure of 610 eyes decreased to≤21 mmHg(1 mmHg=0.133 kPa),and AOD500 increased significantly after LPI(P=0.014).The preoperative iris curvature of 610 eyes were≥0.210 mm.The postoperative intraocular pressure were still>21 mmHg in 253 eyes,but the postoperative AOD500 increased slightly(P=0.083).The preoperative iris curvature of 253 eyes was 0.197-0.209 mm.The postoperative IOP were not changed in 57 eyes and the difference of AOD500 was not statistically significant before and after LPI(P=0.558).The preoperative iris curvature of 57 eyes were 0.076-0.196 mm.Conclusion Early PACG with preoperative iris curvature≥0.210 mm is suitable for LPI.The postoperative AOD500 is significantly increased and the antihypertensive effect is significant.The higher the preoperative iris curvature,the more obvious the increase of AOD500 after operation,and the better the intraocular pressure reduction effect.
