老年性白内障术前仰卧位散瞳药滴眼的效果

摘　　要：目的:评估老年性白内障患者术前仰卧位散瞳药滴眼的散瞳效果。方法:前瞻性研究。选取焦作煤业集团中央医院2012年4月至2014年5月老年性白内障手术204例(204眼),随机分为仰卧位组和坐位组,每组102例。仰卧位组患者采取仰卧位滴散瞳药,坐位组患者采取坐位头尽量往后仰滴散瞳药。分别于散瞳前、首次滴眼散瞳后30分钟和晶状体植入前对瞳孔直径进行测量,记录两组患者散瞳前后瞳孔直径之差,比较两组患者散瞳达标率。结果:仰卧组患者首次滴眼散瞳后30分钟和晶状体植入前瞳孔直径分别为(8.07±0.71)mm和(8.25±0.70)mm,散瞳前后直径之差为(4.51±0.78)mm,均大于坐位组(均P<0.05);仰卧位组患者达标率和基本达标率分别为76.9%和23.1%,与坐位组相比差异具有统计学意义(P<0.05)。结论:老年性白内障患者术前仰卧位散瞳药滴眼可提高散瞳效果,增加术前散瞳达标率。Objective To evaluate the efficacy of mydriatic eye drops in supine position before operation for senile cataract.Methods This was a prospective study.Total of 204 eyes of 204 cases of senile cataract required preoperative dilation were randomly divided into the supine group and the sitting group,with each group of 102 cases.The supine group received mydriatic eye drops in supine position,and the sitting group received mydriatic drops in sitting posture,with head thrown back as much as possible.Pupil diameter was measured before pupil dilation,30 minutes after the first mydrasis and before IOL implantation.The difference in pupil diameter before and after mydriasis in the two groups was recorded.The compliance rate of pupil dilation was compared between the two groups.Results The pupil diameter in the supine group after the first 30 min and before IOL implantation were(8.07±0.71)mm and(8.25±0.70)mm,the difference of diameter before and after dilation was(4.51±0.78)mm,which was larger than that in sitting group(all P<0.05).The basic compliance rate in the supine position was 76.9%and 23.1%in the sitting group,respectively.The difference was statistically significant between two groups(P<0.05).Conclusion Preoperative mydriatic eye drops in supine position can increase the efficacy of mydriais of senile cataract patients and increase dilation compliance rates preoperatively.
