抵当汤联合利伐沙班治疗恶性肿瘤并发深静脉血栓形成临床观察

摘　　要：目的:探讨中药方剂抵当汤联合利伐沙班对恶性肿瘤合并深静脉血栓形成(DVT)患者临床治疗的有效性与安全性。方法:选取山东中医药大学附属医院2016年3月至2019年3月收治的恶性肿瘤并发DVT患者,用随机数字表法分为治疗组和对照组各60例,分别给予抵当汤联合利伐沙班和利伐沙班单独进行治疗。治疗30d,观察两组下肢周径差、凝血指标、临床疗效、DVT复发率及出血等不良事件,并进行统计学分析。结果:治疗组下肢周径差、凝血指标改善优于对照组,差异有统计学意义(P<0.05);两组在临床疗效、DVT复发率及出血等不良事件方面比较,差异均无统计学意义(P>0.05)。结论:抵当汤联合利伐沙班治疗恶性肿瘤合并DVT,能减轻肢体肿胀程度,改善凝血指标,且未增加药物不良反应发生风险,安全、有效。Objective:To investigate the efficacy and safety of the combination of Chinese formula Didang Decoction(抵当汤)and Rivaroxaban in the clinical treatment of malignant tumor complicated with deep venous thrombosis(DVT).Methods:Patients with malignant tumor complicated with DVT admitted to the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from March 2016 to March 2019 were selected and divided into the treatment group and the control group by random number table method,total 60 patients in each group were given the combined Didang Decoction and Rivaroxaban and Rivaroxaban respectively.After 30 days of treatment,the circumference difference of lower limb,coagulation indexes,clinical efficacy,DVT recurrence rate and bleeding adverse events in the two groups were observed and statistically analyzed.Results:The circumference difference of lower limb and coagulation index improvement of the treatment group were better than those of the control group,and the difference was statistically significant(P<0.05).There were no statistically significant differences between the two groups in terms of clinical efficacy,DVT recurrence,bleeding and other adverse events(P>0.05).Conclusions:Didang Decoction combined with Rivaroxaban in the treatment of patients with malignant tumors and DVT can reduce the degree of limb swelling and improve the blood coagulation index without increasing the risk of adverse reactions.The treatment is safe and effective.
