乙肝肝硬化患者上消化道病变胃镜检出结果分析

摘　　要：目的探讨乙肝肝硬化患者上消化道病变的胃镜检出情况。方法选取2018年1月~2019年1月我院收治的乙肝肝硬化患者243例,按诊断分为失代偿期组135例,代偿期组108例;收集患者的一般资料、胃镜检查结果、胃黏膜组织病理检查结果,比较两组反流性食管炎(RE)、消化性溃疡(PU)、食管静脉曲张(EV)、慢性萎缩性胃炎(CAG)的检出情况。结果RE、PU、EV、CAG的总检出率分别为10.29%、20.16%、78.60%、84.77%;失代偿期组RE、PU、EV及中重度EV、CAG及中重度CAG的检出率均高于代偿期组,差异有统计学意义(P<0.05)。结论乙肝肝硬化合并RE、PU、EV、CAG临床上多见,RE、PU、EV及中重度EV、CAG及中重度CAG的发生率与肝硬化程度密切相关。Objective To explore the gastroscopy detection of upper gastrointestinal lesions in patients with hepatitis B liver cirrhosis.Methods A total of 243 patients with hepatitis B cirrhosis admitted to our hospital from January 2018 to January 2019 were selected and divided into 135 cases in the decompensated group and 108 cases in the compensated group according to the diagnosis;general information and gastroscopy results of the patients were collected 2.Pathological examination results of gastric mucosa,comparing the detection of reflux esophagitis(RE),peptic ulcer(PU),esophageal varices(EV),and chronic atrophic gastritis(CAG)between the two groups.Results The total detection rates of RE,PU,EV,and CAG were 10.29%,20.16%,78.60%,and 84.77%,respectively;the detection rates of RE,PU,EV and moderate to severe EV,CAG,and moderate to severe CAG in the decompensation group all were higher than the compensation period group,the difference was statistically significant(P<0.05).Conclusion Hepatitis B liver cirrhosis with RE,PU,EV,and CAG was more common in clinical practice.The incidence of RE,PU,EV and moderate to severe EV,CAG,and moderate to severe CAG was closely related to the degree of cirrhosis.
