摘　　要：目的探讨晚蚕沙外熨联合针对性干预对糖尿病性周围神经病变患者临床疗效及生活质量的影响。方法将2018年5月—2019年4月本科收治的180例糖尿病性周围神经病变寒凝血瘀型患者根据入院时间分为对照组(90例)与观察组(90例)。对照组患者给予口服甲钴胺治疗,观察组患者在对照组治疗基础上联用晚蚕沙外熨及针对性干预治疗。两组患者4周为1个疗程,评估各组治疗前后临床疗效及生活质量量表(SF-36)评分。结果(1)两组治疗后肢体麻木、肢体疼痛、手足畏寒发凉的症状积分较治疗前均明显好转(P<0.05);(2)两组中医主症总疗效比较:观察组及对照组总有效率分别为81.18%、66.27%,与对照组比较差异有统计学意义(P<0.05);(3)与对照组比,观察组生活质量改善显著,差异有统计学意义(P<0.05)。结论晚蚕沙外熨联合针对性干预可有效改善糖尿病周围神经病变患者症状、提高疗效并且改善患者的生活质量。Objective To investigate the effect of combined treatment of night silkworm and external nursing on the clinical efficacy and quality of life of patients with diabetic peripheral neuropathy.Methods From May 2018 to April 2019,180 cases of diabetic peripheral neuropathy with cold blood stasis were divided into control group(90 cases)and observation group(90 cases)according to the admission time.The control group was treated with oral Mecobalamin,and the observation group was treated with external ironing of silkworm excrement and targeted intervention on the basis of the control group.The clinical efficacy and SF-36 scores were evaluated before and after treatment.Results(1)The symptom scores of limb numbness,limb pain,chills and colds of the hands and feet after treatment were significantly better than those before treatment(P<0.05).(2)Comparison of the total efficacy of the two groups of TCM patients:observation group and The total effective rate of the control group was 81.18%and 66.27%,which was statistically significant compared with the control group(P<0.05).(3)Compared with the control group,the quality of life of the observation group improved significantly,and the difference was statistically significant(P<0.05).Conclusion Late silkworm external ironing combined with targeted nursing intervention can effectively improve the symptoms of patients with diabetic peripheral neuropathy,improve the efficacy and improve the quality of life of patients.
