摘 要:目的探讨卵巢肿块超声造影定量参数与血管内皮生长因子(VEGF)和缺氧诱导因子(HIF-1α)的相关性。方法选取2014年1月至2016年12月间西安医学院第二附属医院收治的100例发现卵巢肿块的患者,包括良性56例和恶性44例。术前均行超声造影检查,术后病理标本行免疫组化染色检测,分析超声造影参数诊断法和病理诊断法的一致性及相关性。结果良性肿块中,48例显示内部低增强,包括8例无增强;8例显示内部高增强。恶性肿块中,30例显示内部高增强,14例显示内部低增强。卵巢恶性肿块的造影剂上升时间、达峰时间、峰值强度和平均渡越时间与良性结节比较,差异均有统计学意义(均P <0.05)。参数成像诊断法计算卵巢肿块的敏感性、特异性和约登指数分别为86.4%、89.3%和0.76。VEGF在卵巢良性肿块组织中阳性表达率为14.3%,在卵巢恶性肿块组织中阳性表达率为72.7%,差异有统计学意义(P <0.05)。HIF-1α在卵巢良性肿块组织阳中性表达率为10.7%,在卵巢恶性肿块组织中阳性表达率为68.2%,差异有统计学意义(P <0.05)。经Spearman相关分析,卵巢恶性肿块组织中,HIF-1α与VEGF表达呈正相关(P <0.05)。超声造影定量参数达峰强度与卵巢良恶性肿块VEGF和HIF-1α表达呈正相关,上升时间、达峰时间和平均渡越时间与卵巢良恶性肿块VEGF和HIF-1α表达呈负相关。结论卵巢肿块超声造影定量参数与其VEGF和HIF-1α具有一定相关性。Objective To explore the synchronous correlation between parameters used in contrastenhanced ultrasound and expression of vascular endothelial growth factor( VEGF) and HIF-1 alpha for ovarian masses. Methods One hundred patients with ovarian mass treated at The Second Affiliated Hospital of Xi’an Medical University were selected from January 2014 to December 2016 including 56 patients with benign tumors and 44 patients with malignant tumors. Contrast-enhanced ultrasound was performed on all the included patients before the surgery. After surgery,the immunohistochemical staining of pathological specimens was detected. The consistency and correlation between angiographic parameter and pathologic diagnosis were analyzed. Results Low internal enhancement was observed in 48 patients with benign tumors including 8 patients without enhancement and high internal enhancement was observed in 8 patients. In patients with malignant tumors,high internal enhancement was observed in 8 patients and low internal enhancement was observed in 14 patients. There was significant difference in rise time,time to peak,maximum intensity and mean transit time for contrast medium between the two groups( P > 0. 05). Sensitivity,specificity and Youden index of parametric imaging diagnosis for ovarian masses was 86. 4%,89. 3% and0. 76,respectively. The positive expression rate of VEGF was 14. 3% in benign tumor tissues and 72. 7%in malignant tumor tissues( P < 0. 05). Spearman analysis revealed that the expression of VEGF was positively associated with the expression of HIF-1α in malignant ovarian tumor tissues( P < 0. 05).
