摘　　要：目的探讨CXCL12在多发性骨髓瘤骨髓中表达及与预后的相关性。方法选择多发性骨髓瘤骨髓活检标本78例(观察组)与正常骨髓活检标本78例(对照组),采集所有患者的骨髓,采用免疫组化法检测骨髓CXCL12表达水平。调查所有患者的一般资料,随访预后并进行影响因素分析。结果观察组的骨髓CXCL12表达阳性率为85.9%,显著高于对照组的12.8%(P<0.05)。在78例多发性骨髓瘤患者中,的骨髓CXCL12表达阳性率与ISS分期、危险分层、轻链类型呈正相关关系,差异有统计学意义(P<0.05),而与性别、年龄、免疫球蛋白分型等无相关性(P>0.05)。观察组患者随访到2019年8月,死亡病例21例,存活病例57例,死亡率为26.9%。多因素分析结果显示,ISS分期、危险分层、轻链类型、CXCL12等都是影响多发性骨髓瘤预后死亡的因素(P<0.05)。结论多发性骨髓瘤患者中的骨髓CXCL12呈现高表达状况,且与患者的病理特征显著相关,也是影响患者预后的重要因素。Objective To investigate the expression of CXCL12 in bone marrow of multiple myeloma and correlation with prognosis.Methods 78 patients with multiple myeloma bone marrow biopsy specimens(the observation group)and seventy-eight cases of normal bone marrow biopsy specimens(the control group)were selected.The expression level of bone marrow CXCL12 was detected by immunohistochemistry.Investigate the general information of all patients,follow up the prognosis and analyze the influencing factors.Results The positive rates of CXCL12 expression in the observation group was 85.9%,which was significantly higher than that in the control group(12.8%,P<0.05).In the 78 patients with multiple myeloma,the positive rates of bone marrow CXCL12 expression was positively correlated with ISS stage,risk stratification,and light chain type,and the difference were statistically significant(P<0.05),but with gender,age,and immune ball,there were no correlation between protein typing(P>0.05).Patients in the observation group was followed up to August 2019,there were 21 cases were died,there were 57 cases were survived,and the mortality rates was 26.9%.Multivariate analysis showed that ISS stage,risk stratification,light chain type,and CXCL12 were all factors that affected the prognosis of multiple myeloma(P<0.05).Conclusion Bone marrow CXCL12 is highly expressed in patients with multiple myeloma,and is significantly associated with the pathological features of patients,and is also important factors affecting the prognosis of patients.
