宫颈TCT联合电子阴道镜检查在宫颈阳性病变患者诊断中的应用价值分析

摘　　要：目的分析在疑似宫颈筛查为阳性的病变患者诊断中应用宫颈薄层液基细胞技术(TCT)联合电子阴道镜检查的临床价值。方法方便选取该院妇科门诊自2015年12月—2019年12月以来收治的491例疑似宫颈筛查为阳性的病变患者当作研究对象,所有患者均应用宫颈TCT和电子阴道镜进行检查,同时对所有受检者宫颈多位点组织进行活检并送病理检查,然后将病理检查结果作为金标准,统计并分析宫颈TCT和电子阴道镜在检查宫颈阳性病变患者中的诊断效能。结果在491例患者中,宫颈TCT检出59例异常者,诊断宫颈上皮内瘤变(CIN)的敏感性、特异性、准确率分别为59.21%(45/76)、96.63%(401/415)、93.69%(460/491);电子阴道镜检出81例异常者,诊断CIN的敏感性、特异性、准确率分别为93.42%(71/76)、97.59%(405/415)、98.98%(486/491);宫颈TCT联合电子阴道镜检查对诊断CIN的敏感性、特异性、准确率分别为96.05%(73/76)、96.14%(399/415)、99.18%(487/491)。结论在疑似宫颈筛查为阳性的病变患者诊断中应用宫颈TCT联合电子阴道镜检查具有较高的临床价值,能够发现早期宫颈病变。Objective To analyze the clinical value of cervical thin layer liquid-based cell technology(TCT)combined with electronic colposcopy in the diagnosis of patients with suspected positive cervical screening.Methods A total of 491 patients with suspected cervical screening positive from December 2015 to December 2019 in the gynecological clinic were convenient selected as subjects.All patients were examined by cervical TCT and electronic colposcopy.The biopsy of the examiner's cervical multi-site tissue was sent for pathological examination,and then the pathological examination results were used as the gold standard to count and analyze the diagnostic efficacy of cervical TCT and electronic colposcopy in patients with positive cervical lesions.Results Among 491 patients,59 patients with abnormalities were detected by cervical TCT.The sensitivity,specificity and accuracy of diagnosis of cervical intraepithelial neoplasia(CIN)were 59.21%(45/76)and 96.63%(401/415),93.69%(460/491);81 cases of abnormalities detected by electronic colposcopy,the sensitivity,specificity and accuracy of diagnosis of CIN were 93.42%(71/76),97.59%(405/415),98.98%(486/491);the sensitivity,specificity and accuracy of cervical TCT combined with electronic colposcopy for the diagnosis of CIN are 96.05%(73/76),96.14%(399/415),99.18%(487/491).Conclusion The use of cervical TCT combined with electronic colposcopy in the diagnosis of patients with suspected positive cervical screening has high clinical value and can detect early cervical lesions.
