IMRT同步化疗治疗食管癌术后食管气管沟淋巴结转移的临床疗效分析

摘　　要：目的探讨调强放射治疗(IMRT)同步化疗治疗食管癌术后食管气管沟淋巴结转移的疗效及不良反应。方法方便选取泰安市肿瘤防治院2015月1月—2018年12月收治的60例食管癌术后食管气管沟淋巴结转移的患者,其中IMRT同步化疗组28例,单纯IMRT组32例,观察两组的近期疗效,远期生存率以及不良反应。结果单纯IMRT组总体有效率62.5%,低于IMRT同步化疗组的85.7%(χ2=4.115,P<0.05);单纯IMRT组1~3年生存率与IMRT同步化疗组1~3年生存率差异无统计学意义(χ2=1.655,P>0.05);单纯IMRT组骨髓抑制发生率为53.13%低于IMRT同步化疗组的78.57%(χ2=4.250,P<0.05);两组病死率差异无统计学意义(χ2=0.044,P>0.05)。结论IMRT同步化疗治疗食管癌术后气管食管沟淋巴结转移,局部控制率好,不良反应在可耐受范围内。Objective To investigate the efficacy and adverse effects of IMRT combined with chemotherapy in the treatment of esophageal tracheal lymph node metastasis after esophageal cancer surgery.Methods 60 patients with esophageal tracheal lymph node metastasis after operation of esophageal cancer from January 2015 to December 2018 in Tai'an Cancer Prevention and Treatment Hospital were convenient selected.Among them,28 were in IMRT synchronous chemotherapy group and 32 were in IMRT alone group.The short-term efficacy,long-term survival rate,and adverse reactions.Results The overall effective rate in the IMRT alone group was 62.5%,which was lower than 85.7%in the IMRT simultaneous chemotherapy group(χ2=4.115,P<0.05);the 1-3 year survival rate of the IMRT simple group and the IMRT 1-3 year survival rate no statistically significant difference(χ2=1.655,P>0.05);the incidence of bone marrow suppression in the IMRT alone group was 53.13%lower than 78.57%in the IMRT simultaneous chemotherapy group(χ2=4.250,P<0.05);there was no statistically significant difference in mortality between the two groups(χ2=0.044,P>0.05).Conclusion IMRT simultaneous chemotherapy for tracheoesophageal sulcus lymph node metastasis after esophageal cancer surgery has good local control rate and adverse reactions are within the tolerable range.
