摘　　要：目的:探讨传统结扎翼托槽与自锁托槽在错[牙合]矫治中的应用效果及对牙周状况的影响。方法:选取2016年7月-2019年12月笔者医院诊治的60例口腔牙齿矫正患者为研究对象,采用随机数字表法分为观察组和对照组,每组30例。对照组:采用传统结扎翼托槽矫治器治疗;观察组:采用自锁托槽矫治器治疗,比较两组患者治疗1年后的总有效率,矫正丝的结扎与拆除时间、治疗前后菌斑指数(PLI),龈沟出血指数(SBI)、临床附着丧失(CAL)、牙龈退缩指数(GR)、牙周袋深度(PD)以及生活质量评分。结果:治疗后,观察组患者的总有效率明显高于对照组(P<0.05);观察组患者牙齿矫正的结扎与拆除时间均低于对照组(P<0.05);治疗后观察组患者的PLI、SBI、PD、CAL指标均明显低于对照组(P<0.05),GR指标和生理性疼痛、功能受限、社交困难、生理障碍方面的生活质量评分均高于对照组(P<0.05)。结论:相比于传统结扎翼托槽,在错[牙合]畸形矫治中应用自锁托槽矫治器,可缩短牙齿矫正结扎与拆除的操作时间,改善牙周情况和生活质量,提高临床疗效。Objective To explore the application effect of tie-wing bracket and self-locking bracket in malocclusion orthodontics and their influences on periodontal status.Methods Sixty patients who underwent oral orthodontics and were admitted to the hospital from July 2016 to December 2019 were enrolled as the research objects.They were randomly divided into observation group and control group by random number table method,30 cases in each group.The control group was treated with traditional tie-wing bracket appliance,while observation group was treated with self-locking bracket appliance.The total response rate after 1 year of treatment,ligation and removal time of orthodontic wire,plaque index(PLI),sulcular bleeding index(SBI),clinical attachment level(CAL),gingival recession(GR),pocket depth(PD)and quality of life score before and after treatment were compared between the two groups.Results After treatment,total response rate in observation group was significantly higher than that in control group(P<0.05).The ligation and removal time of orthodontics in observation group were shorter than those in control group(P<0.05).After treatment,PLI,SBI,PD and CAL in observation group were significantly lower than those in control group(P<0.05),while GR index and quality of life scores in terms of physiological pain,functional limitation,social difficulties and physical disorders were higher than those in control group(P<0.05).Conclusion Compared with traditional tie-wing bracket,the application of self-locking bracket appliance in malocclusion orthodontics can shorten the operation time of orthodontics ligation and removal,improve periodontal situations,quality of life and clinical curative effect.
