正畸种植联合矫治前牙缺失伴牙列不齐疗效观察

摘　　要：目的:探讨正畸种植联合矫治前牙缺失伴牙列不齐的疗效,以及对患者美观满意度的影响。方法:选取2016年10月-2018年10月笔者医院收治的前牙缺失及牙列不齐患者57例,予以牙体种植联合正畸矫治。观察患者临床疗效、种植成功率、种植成功者牙齿咬合力,比较治疗前后满意度、炎症相关因子[前列腺素(PGE2)、基质金属蛋白酶-2(MMP-2)]。结果:57例患者经正畸种植联合矫治后,总有效率为94.74%,共种植68颗牙齿,牙体种植治疗后3个月、6个月、1年,种植成功率分别为92.65%、88.24%、79.41%,牙齿咬合力分别为(543.19±30.27)N、(548.32±31.05)N、(553.41±33.07)N,随着时间推移,呈平稳上升趋势;种植治疗6个月后,57例患者咀嚼功能、语言功能、舒适度及美观度的满意度评分均较治疗前升高(P<0.05),龈沟液PGE2、MMP-2、ICAM-1水平均较治疗前降低(P<0.05)。结论:正畸种植联合矫治前牙缺失伴牙列不齐的疗效显著,牙体种植成功率较高,患者具有较强的牙齿咬合力,牙周炎症减轻,而咀嚼功能、语言功能、舒适度和美观满意度有显著提高。Objective To explore the efficacy of orthodontics combined with implantation on the correction of anterior teeth loss with malocclusion and the effects on aesthetic satisfaction of patients.Methods 57 patients with anterior teeth loss and malocclusion treated in our hospital from October 2016 to October 2018 were selected,and they were given dental implantation combined with orthodontics for correction.The clinical efficacy,implantation success rate,and dental occlusion strength of patients with successful implantation were observed among the patients,and the satisfaction and inflammation-associated cytokines[prostaglandin(PGE2),matrix metalloproteinase-2(MMP-2)]were compared before and after treatment.Results After correction with orthodontics combined with implantation for 57 patients,the total effective rate was 94.74%,and a total of 68 teeth were implanted.At 3 months,6 months and 1 year after dental implantation treatment,the implantation success rates were 92.65%,88.24%and 79.41%respectively,and the dental occlusion strength were(543.19±30.27)N,(548.32±31.05)N and(553.41±33.07)N,with a steady upward trend over time.After 6 months of implantation treatment,the satisfaction scores of chewing function,language function,comfort and aesthetics of 57 patients were all higher than those before treatment(P<0.05),and the levels of PGE2,MMP-2 and ICAM-1 in gingival crevicular fluid were lower than those before treatment(P<0.05).Conclusion Orthodontics combined with implantation has a significant efficacy in the correction of anterior teeth loss with malocclusion,and it can make patients have strong dental occlusion strength,can relieve periodontal inflammation and effectively enhance chewing function,language function,comfort and aesthetic satisfaction of patients.
