摘 要:目的评价血清前列腺特异性抗原(PSA)在乳腺癌患者中表达水平的变化及其诊断价值。方法在Pubmed、Embase、The Cochrane library(考克兰图书馆)、中国知网及万方医学数据库中,检索从2010年1月至2020年4月发表的研究乳腺癌中血清PSA浓度以及血清PSA在乳腺癌中的诊断价值的研究,采用Revman 5.3软件进行Meta分析。结果一共纳入了15项研究,包括1 361例乳腺癌患者,1 128例良性乳腺疾病患者和817例健康对照者。结果显示,(1)相对于良性乳腺疾病患者和健康人,乳腺癌患者的总PSA(tPSA)平均升高0.21 μg/L[95%可信区间(CI):0.17~0.26,P<0.01],游离PSA(fPSA)平均升高0.03 μg/L(95%CI:0.02~0.03,P<0.01),游离与总PSA比值(fPSA/tPSA)平均升高0.29(95%CI:0.23~0.35,P<0.01)。(2)根据集成受试者工作特征(ROC)曲线,相对于tPSA和fPSA,fPSA/tPSA在区分乳腺癌和非乳腺癌时诊断效能最强。结论相对于非乳腺癌人群,乳腺癌患者中PSA的表达水平升高。PSA检测,尤其是fPSA/tPSA,可能用于乳腺癌的早期诊断。Objective:To evaluate the expression level of serum prostate specific antigen(PSA)in breast cancer patients and its diagnostic value.Methods:In PubMed,EMBASE,the Cochrane Library,CNKI and Wanfang medical database,the research on serum PSA concentration in breast cancer and the diagnostic value of serum PSA in breast cancer published from January 2010 to April 2020 were systematically searched.Two evaluators independently selected literature,extracted data and evaluated quality according to the entry criteria.Revman 5.3 software was used for Meta-analysis.Results:A total of 15 studies were included,including 1361 breast cancer patients,1128 benign breast disease patients and 817 healthy controls,a total of 3306 cases.The results showed that:(1)Compared with benign breast disease patients and healthy people,the average increase of total PSA(tPSA)in breast cancer patients was 0.21μg/L[95%confidence interval(CI):0.17-0.26,P<0.01],the average increase of free PSA(fPSA)was 0.03μg/L(95%CI:0.02-0.03,P<0.01),and the average increase of fPSA/tPSA was 0.29(95%CI:0.23-0.35,P<0.01).(2)According to the integrated receiver operating characteristic(ROC)curve,compared with tPSA and fPSA,fPSA/tPSA is the most effective in the diagnosis of breast cancer.Conclusion:Compared with non-breast cancer population,PSA expression level in breast cancer patients increased.PSA detection,especially fPSA/tPSA,may be used in the early diagnosis of breast cancer.
