摘　　要：目的分析乳腺癌瘤周水肿与预后因子之间的相关性.方法选取行乳腺MRI检查并经穿刺活检或手术病理证实的乳腺癌患者72例,由2名影像诊断医师结合T2 WI及动态增强序列共同判断肿瘤周围是否存在水肿,并与雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体-2(HER2)、Ki-67表达状态及病理分级、分子分型对照分析.结果72例乳腺癌中,35例可见瘤周水肿(48.6%).瘤周水肿的发生率在不同ER、PR、Ki-67表达水平及分子分型乳腺癌之间有统计学差异(P<0.05),瘤周水肿多见于ER阴性、PR阴性、Ki-67高表达以及三阴性乳腺癌(TNBC)患者中.瘤周水肿在HER2高表达和低表达之间无统计学差异(P>0.05).结论乳腺癌瘤周水肿与ER/PR、Ki-67及分子分型存在一定的相关性,术前MRI观察瘤周水肿情况对判断肿瘤的侵袭性、评估患者的预后有指导意义.Objective To analyze the correlation between peritumoral edema and prognostic factors of breast cancer.Methods 72 patients with breast cancer underwent MRI and confirmed by puncture biopsy or surgical pathology were selected.MR images of the patients were reviewed by the two radiologists,and peritumoral edema were detected according to the T2WI and dynamic contrast-enhanced images.Expression of the breast cancer prognostic factors(ER,PR,HER2,Ki-67)were obtained by immunohistochemistry.Molecular subtypes and pathological grading were also categorized.The correlation between peritumoral edema and prognostic factors of breast cancer was analyzed.Results Among the 72 breast cancers,peritumoral edema was found in 35 patients(48.6%).Peritumoral edema was more frequently found in patients with negative ER statue,negative PR statue,higher Ki-67 index,and triple-negative breast cancer(TNBC)(all P<0.05).There was no statistical difference between high expression and low expression of HER2 in peritumoral edema(P〉0.05).Conclusion There is a certain correlation between peritumor edema and expression of ER/PR,Ki-67 index and molecular subtype.Peritumoral edema assessment on MRI could provide valuable information for patients with invasive breast cancer.
