摘　　要：目的研究MSCT对胆囊癌神经周围侵犯术前评估的价值.方法回顾性分析41例经病理证实的胆囊癌患者的影像及临床资料.采用独立样本t检验及χ^2检验分析有、无神经周围侵犯2组患者中肿瘤最大径、T分期、N分期以及胆囊管侵犯的差异性.采用逻辑回归分析神经周围侵犯与上述影像征象的相关性.结果有神经周围侵犯组的T分期(P=0.030)、胆囊管侵犯比率(P<0.001)均显著高于无神经周围侵犯组.2组患者在肿瘤最大径和N分期无统计学差异(P>0.05).肿瘤T分期(OR=5.704,95%CI=1.20~27.12,P=0.029)及胆囊管侵犯(OR=31.778,95%CI=5.57~181.23,P<0.001)均与神经周围侵犯显著相关.结论MSCT的胆囊癌T分期及胆囊管侵犯与胆囊癌神经周围侵犯显著相关;提示这2种征象可能是术前判断胆囊癌神经周围侵犯的重要影像学特征.Objective To investigate the value of MSCT in preoperative evaluation of perineural involvement of gallbladder carcinoma.Methods Imaging and clinical data of 41 patients histologically diagnosed as gallbladder carcinoma were analyzed retrospectively.Independent-samples t test and Chi-square test were used to analyze the differences of the maximum tumor diameter,T-stagingf N-staging and involvement of cystic duct between patients with and without perineural involvement.LogtJitc regression was used to assess the association between perineural involvement and above imaging features.Results The T-staging(P=0.030)and involvement of cystic duct(P<0.001)in patients with peripheral involvement were significantly higher than those in patients without peripheral involvement.There were not significant differences in the maximum tumor diameter and N-staging between two groups(all P>0.05).The T-staging(OR=5,704,95%CI=1.20-27.12,P=0.029)and involvement of cystic duct(OR=31.778,95%CI=5.57-181.23,P<0.001)were significantly associated with perineural mvolvement.Conclusion The T-staging and cystic duct involvement of gallbladder carcinoma on MSCT are significantly associated with perineural involvement suggesting that these two features might be important radiologic characteristic for preoperatively diagnosing perineural involvement of gallbladder carcinoma.
